On 05/05/2026 Licensing Program Analyst (LPA) Jillinda Chandler made an announced visit to the Footsteps child care center for the purpose of conducting a prelicensing inspection. The center is a two-story building with daycare activity being conducted on the lower level only. The center has a combination of three addresses 5517 (lower level on the east side of the building). This address will be used for preschool children. There were 3 classrooms that will be used for preschool activities; one inside the main building and 2 two detached rooms in the rear (previously garages). The 5519 is the upper level over 5517 that will be off limits to children, and 5521 will be dedicated to preschool activities. At 5521 there was one large area (near the reception area )within this area is a glass enclosed area that can be separated into two classrooms.

The applicant is requesting a capacity of 89 children, 45 preschool children ages 2 – 6 years, and 44 toddlers ages 18 -36 months. LPA met with Kristin Chamberlin (applicant) and Mercy Thomas (director) who provided a tour of the facility. Daycare will be conducted Monday – Friday/ 8:00 a.m. – 5:00 p.m. There is an approved fire clearance on file conducted on 12/8/2025 by Inspector Lawrence Kim of L.A City Fire department.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following was observed of the:

INDOOR ACTIVITY SPACE

Fire extinguishers were 2AB10C or larger, last serviced 1/13/2026

Carbon monoxide detectors were not observed in several of the classrooms. Applicant shall install carbon monoxide detectors. The center is equipped with a fire alarm system

First aid kits were available with the required essentials: scissors, bandages, tweezers, ointments, manuals and thermometer.

Age-appropriate furniture and equipment was observed in good repair. LPA observed toys for toddlers that provided auditory, visual, tactile stimulation and manipulative skills

Cots were observed for napping. Director was informed that bedding must be removed and stored separately when cots are stacked.

Cubbies were observed for children’s belongings.

Drinking water will be provided through filtered water stations.

The facility has heating and cooling units in the rear classrooms and central heating and cooling in the main buildings. LPA observed an fire place in the preschools outdoors area, this shall be made inaccessible to children in care, there were no open face heaters.

Windows were in good repair free of chipping paint, dirt, insects, or debris.

Adequate lighting was observed.

LPA observed trash cans used for solid waste with tight-fitting lids.


Disinfectants, cleaning solutions, toxins or other poisons were observed inaccessible to children in care.

The director’s office will be used for isolation of ill children, these children will use their dedicated restroom with sanitizing after each use. Applicant was informed that there must be a cot, mat, or crib readily available for ill children and that these children must be under direct visual supervision at all times.

The facility was not equipped with a working telephone. Per the applicant cell phones will be used, LPA explained there shall be a working cell phone primarily for the daycare center at all times.

The classrooms were clean in good repair.

Parents and authorized adults will sign children in and out, using an electronic signing device. The applicant was informed that sign-in sheets must be maintained for a minimum of 30 days and readily available for review at the request of the department or its representative.

The required postings were not posted; applicant was advised that the postings must be posted in a prominent area for viewing at all times during daycare operations. LPA provide the LIC. 311D - Records to be maintained at the facility.

Per the applicant there are cameras on the outside of the center

The director was informed that parents shall give written permission for the placement of their children in the toddler age component, with the written permission maintained in the facility’s file for the child.

FOOD SERVICE:

Meals and snacks will be provided by parents. Applicant was informed there must be an adequate supply of foods readily available in the case of emergencies.

LPA observed a kitchen on the upper level of the building. With a sink, storage for foods and a refrigerator with a thermometer.

Center has not devised an Incidental Medical Service (IMS) plan for children with allergies (epi-pen), asthmatic (inhalers) glucose monitoring (diabetics), and children needing gastrostomy tube (g-tube) feeding. IMS was discussed with the applicant.

5 sinks and 6 toilets available for children use
applicant shall provide 3 or more potty chairs to meet the toilet requirement for the toddler program.
The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold water. Changing tables were within reach of a sink, padding surfaces were at least one-inch thick, covered with washable vinyl or plastic with raised sides at least three inches high.

OUTDOOR ACTIVITY SPACE

Age-appropriate toys and equipment were observed in good condition.

The outdoor activity space was enclosed with a 4 foot or higher gate/wall.

Resilient cushioning and artificial grass was observed in good condition.

Children will use their personal water bottles for drinking water

LPA observed shading, there were no benches for resting on the toddler yard, applicant shall provide resting equipment on this side

LPA did not observe any bodies of water during today’s inspection. Director was informed that all standing water should be removed immediately after each use.

Applicant shall fill the water basin (tub) on the toddler yard or make it inaccessible to children, fill the hole on the preschools bike area, add a latch to the exit gate (not a pad lock or dead bolt) applicant shall ensure storage area under the stair case is made inaccessible to children in care.

Applicant shall ensure that entries into the detached classroom do not cause a tripping hazard.

MEASUREMENTS:



Indoor Activity Space:
Preschool measurements equaled 1538.86 square feet, which will not accommodate the requested capacity the measurements were rounded up to allow the a maximum capacity of 44.

Toddler’s measurement equaled 2175.21 square feet, which will accommodate the requested capacity.

Outdoor Activity Space:
Preschool measurements equaled 1766.27 square feet, for a total of 23 children utilizing this space at any one time. This will not accommodate the requested capacity.

Toddlers outdoor measurements equaled 1171.95 for a total of 15 children at any on time utilizing this space.

Applicant shall request a waiver for the preschool and toddler yard of Title 22, section 101238.2(a)- There shall be at least 75 square feet per child of outdoor activity space based on the total licensed capacity

Based on today’s inspection, the facility shall be recommended for a maximum capacity of 88 children. 44 preschool children determined by the indoor measurements and 44 toddler children determined by the requested capacity.
Licensure will be pending corrections and waiver approvals

applicant was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP.
