On 12/23/25 at 1PM, Licensing Program Analyst (LPA) Angela Luz conducted an announced pre-licensing/ change of location inspection with Licensee Li Li.  The purpose of this inspection is to ensure the standards for a Family Child Care Home (FCCH) are being met in accordance with California Title 22 Regulations and California Health and Safety Codes.

This application is for a change of location. Previous facility number is 197494919 located at 6434 W 87th Street Los Angeles CA, 90045. Licensee is applying for a Large Family FCCH with a maximum capacity of 14 at this address. Ages to be served are 2 years old to 5 years old. Hours of operation are Monday to Friday from 8AM to 5PM. Applicant was informed that any changes to ages, hours and days of operation shall be submitted to the department for approval prior to initiation of changes.

LPA toured the whole home with Licensee inside and outside. The facility is a story single-family home with 4 bedrooms, 2 bathrooms, a living room, dining room, kitchen, family room, laundry room, backyard, and garage. The home is neat and clean with heating and ventilation for safety and comfort.
ON Limit areas: Family room, bedroom #4, bathroom #2, laundry room, and backyard.
OFF Limit areas: Bedrooms #1-3, bathroom #1, kitchen, and garage.

LPA observed a sliding partition wall between the kitchen and the family room. The sliding partition will remain closed during operating hours. When the sliding partition wall is closed, children only have access to on limit areas. LPA observed a child lock on the handles, preventing children from accessing other parts of the home. The garage doors are locked.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Families will access the home by walking up the driveway and entering through the family room door. LPA observed the family room and bedroom #4 to be clean and orderly with age appropriate materials. Children will eat in the family room. Licensee tested the smoke detector and carbon monoxide detector in the family room and LPA noted they were in working order. LPA noted that all electrical outlets in on limit areas are covered. LPA reminded that smoking is not allowed on the premises of a FCCH.

In bedroom #4, LPA noted an electric wall heater with knob installed in the wall. When turned on, the air and metal are hot to the touch. Licensee will remove the knob and place a cover on the hole to prevent use and access from children in care. Children will nap in bedroom #4. Families will provide bedding and will wash them once a week. The laundry room is used as a walkway between bedroom #4 and bathroom #2. LPA inspected cots for napping that were stored in the laundry room.

Children will use bathroom #2. LPA observed a toilet, sink, and child locks on all low cabinets. LPA observed the cabinet under the sink contains cleaning supplies. LPA observed the first aid kit and fully charged 3A40BC fire extinguisher stored in the bathroom. LPA reminded that fire extinguishers shall be serviced or newly purchased annually.

Licensee is in the process of moving and informed LPA that they own firearms that have not yet been moved to this address. Once they are moved to this address, Licensee will send LPA pictures of where the firearms are stored, unloaded firearms, ammunition, and that the firearms and ammunition are stored separately. Licensee informed LPA firearms and ammunition will be stored in the attic separately.

LPA observed the backyard play area. There is a large play structure securely installed into the ground, sand box, and grass area. LPA advised Licensee to place padding under the slide of the play structure for added safety. Licensee covers the sandbox at night and will check the contents of the sand box before children use it in the morning. LPA observed several thorny plants around the perimeter of the backyard. Licensee will remove the thorny plants and send pictures to LPA. There are no bodies of water on the premises.

Forms of discipline used by Licensee are talking with the child, redirection, and offering them comforting items such as a stuffed animal or their family picture. Applicant understands that children's personal rights should not be violated, including but not limited to, no corporal punishment, interference with eating, intimidation, or other actions of a punitive nature. Children are to be treated with dignity, and receive safe, healthful, and comfortable accommodations. Licensee understands how to report unusual incidents/injuries. Sick children will be isolated from the group in the area opposite of where the majority is. If the group is playing outside then the sick child will be isolated in the family room and vice versa.

Licensee CPR and First Aid certificate is current and expires 8/2026. Licensee Mandated Reporter Training for Child Care Providers is current and expires on 9/20/26. Applicant was reminded that Mandated reporter Training (AB1207) and CPR/First aid certifications needs to be renewed every two years. A copy of the applicant’s immunization is on file. LPA reminded Licensee that fire/disaster drills must be practiced at least once every 6 months and documented.

Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The licensee provided proof of control of property.

LPA reviewed with licensee the LIC 311D, Forms/Records to Keep in your Family Child Care Home, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.

On this date, 11/25/2025, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms.

Licensee was advised that a Large FCCH license for this address will be approved pending Licensing Program Manager approval and completion of the following corrections:
  • In bedroom #4, remove the knob on the electric wall heater and place a cover on top.
  • In the backyard, remove the thorny plants.
  • When the firearms are moved, send pictures of the unloaded firearms, ammunition and that the firearms and ammunition are stored separately in the attic.
