On 04/30/2026 Licensing Program Analyst (LPA) Jillinda Chandler made an announced visit to the Happy Nook childcare center, located at 1067 S. Fairfax Ave. Los Angeles, CA. 90019 for the purpose of conducting a pre licensing inspection. The center is a single story building with four classrooms; two rooms will be dedicated to infant care; these classrooms are located on the left side of the building near the office. Across from the director’s office there are two classrooms; one classroom dedicated to the preschool and one to the toddler program (nearest to the play yard).

The applicant is requesting a capacity of 50 children, 12 preschool children ages 2 – 4 years, 14 toddlers ages 18 -36 months and 24 infants ages 0 – 2 years. Daycare will be conducted Monday – Friday/ 8:30 a.m.– 4:30 p.m. There is an approved fire clearance on file conducted on 2/12/2025 by Lawrence Kim of the L.A City Fire Department. LPA met with Priscilla Mazon (applicant),Sophie Warczinsky (director), Diego Mazon (non applicant spouse) and Perla Zavala Benitez (consultant) who provided a tour of the facility.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following was observed of the:

INDOOR ACTIVITY SPACE

Fire extinguishers were 2AB10C or larger.

Carbon monoxide and smoke detectors were observed in operable condition.

First aid kits were available with the required essentials: scissors, bandages, tweezers, ointments, manuals and thermometer.

Age-appropriate furniture and equipment was observed in good repair. LPA observed toys for infants and toddlers that provided auditory, visual, tactile stimulation and manipulative skills

Cots were observed for napping. Director was informed that bedding must be removed and stored separately when cots are stacked. Cots were observed in good condition.

Cubbies were observed for children’s belongings. in each classroom.

Drinking water will be provided through filtered water faucets. Facility has completed lead testing for the water used for food prepping and drinking.

The facility has central heating and cooling. No Fireplaces or open face heaters were observed.

Windows were in good repair free of chipping paint, dirt, insects, or debris.

Adequate lighting was observed.

LPA observed trash cans used for solid waste with tight-fitting lids.

Disinfectants, cleaning solutions, toxins or other poisons were observed inaccessible to children in care.

The director’s office and staff restroom will be used for isolation of ill children. Applicant was informed that there must be a cot, mat, or crib readily available for ill children and that these children must be under direct visual supervision at all times.

The facility was equipped with a working telephone.

The classrooms were clean in good repair.

Parents and authorized adults will sign children in and out, using . an electronic devise. The applicant was informed that sign in sheets must be maintained for a minimum of 30 days and readily available for review at the request of the department or its representative.

The required postings were also posted; applicant was advised that the postings must be posted in a prominent area for viewing at all times during daycare operations.

The director was informed:

· Parents shall give written permission for the placement of their children in the toddler age component, with the written permission maintained in the facility’s file for the child.


· A ratio of six children to each teacher shall be maintained for all children in attendance in the toddler component. An aide who is participating in on-the-job training may be substituted for a teacher when directly supervised by a fully qualified teacher.
· The maximum group size, with two teachers, or one fully qualified teacher and one aide, shall not exceed 12 toddlers.
· The licensee shall maintain compliance with all other regulations pertaining to preschool age children.

The following was discussed regarding infant care (ref. Title 22, subchapter 2)

· There shall be a ratio of one teacher for every four infants in attendance.


· Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant including the LIC. 9227 Individual Infant Sleeping Plan
· Staff shall place infants up to 12 months of age on their backs for sleeping. An infant shall not be swaddled while in care. An infant's head shall not be covered while sleeping. If an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible. Children shall not commingle.

FOOD SERVICE:

Meals will be provided by parents, and the center shall provide morning and afternoon snacks. Applicant was informed there must be an adequate supply of foods readily available in cases of emergencies.

LPA observed a prepping area, with a sink, storage for foods and a refrigerator with a thermometer, toxins or chemicals were stored separately, and LPA did not observe foods during todays inspection. Applicant was advised to add a alerting device to the kitchen door.

Center has devised an Incidental Medical Service (IMS) plan for children with allergies (epi-pen), asthmatic (inhalers) glucose monitoring (diabetics), and children needing gastrostomy tube (g-tube) feeding. IMS was discussed with the applicant.

RESTROOMS

THERE WERE:


9 sinks and 5 toilets available for children use. Three sinks and one toilet are located in the outdoor activity area.
The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold water. Changing tables were within reach of a sink, padding surfaces were at least one inch thick, covered with washable vinyl or plastic, with raised sides at least three inches high.

Applicant was informed there shall be a written toilet-training plan for infants being toilet trained per Title 22, section 101428 - Infant Care Personal Services

OUTDOOR ACTIVITY SPACE

Age-appropriate toys and equipment were observed in good condition.

The outdoor activity space was enclosed with a 4 foot or higher gate/wall.

Sand and artificial grass were observed for cushioning. The sand shall be maintained by a contracted vendor. LPA advised that the sandbox located within the bike run be cushioned

Water will be made readily available outdoors using filtered water pitchers and children use their personal water bottles for drinking.

LPA observed adequate shading and benches for resting.

LPA did not observe any bodies of water during today’s inspection. Director was informed that all standing water should be removed immediately after each use.

No hazardous conditions were observed during todays inspection.

A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA referred applicant to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.

LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
