25 | Licensing Program Analyst (LPA) Antonia Alvizar conducted an Annual Required visit and inspection of the facility. Upon arrival, LPA met Program Director, Marinela Gaitan- Yammine shortly after Administrator, Edward Parker arrived and the purpose of the visit was explained. The program is vendor through the Frank D. Lanterman Regional Center. This day program is licensed for 60 ambulatory Developmentally Disabled Adults, ages 18 through 59. There are 30 clients attending the program today. LPA Alvizar utilized the Compliance and Regulatory Enforcement (CARE) tools.
At 9:30 a.m., LPA Alvizar toured the facility with Mrs. Gaitan- Yammine ensure there are no health and safety hazards. Required postings and readily available for review were observed on the hallway. Smoke alarms and carbon monoxide detectors are interconnected also tested and function properly. There were fire extinguishers observed to be fully charged in the facility and last serviced on 08/09/2023. The last fire drill was conducted on 10/10/2023. The following was observed:
Structure: The program is housed in a single story structure on the corner of a semi-heavy traffic avenue and alley behind it, with parking lot and consists of a reception area, three (3) offices, three (3) storage rooms, five (5) program breakout rooms, locker area for client personal storage, kitchen, a total of four (4) bathrooms; two (2) designated for staff and two (2) designated for clients. There's no bodies of water on the premises. All outdoor and indoor passageways are free of obstruction.
Kitchen: The kitchen appliances and fixtures were functional. Knives, cutlery and other sharp kitchen utensils are stored and locked in the kitchen cabinet. LPA found a sufficient amount of perishable and non-perishable food at Day Program properly stored. There is one (1) commercial size refrigerator in the kitchen area. Dishes, cups and flat ware are stored in the kitchen cabinet, inspected and in good repair. |
