Licensing Program Analyst (LPA) Antonia Alvizar conducted an Annual Required visit and inspection of the facility. Upon arrival, LPA met Program Director, Marinela Gaitan- Yammine shortly after Administrator, Edward Parker arrived and the purpose of the visit was explained. The program is vendor through the Frank D. Lanterman Regional Center. This day program is licensed for 60 ambulatory Developmentally Disabled Adults, ages 18 through 59. There are 30 clients attending the program today. LPA Alvizar utilized the Compliance and Regulatory Enforcement (CARE) tools.

At 9:30 a.m., LPA Alvizar toured the facility with Mrs. Gaitan- Yammine ensure there are no health and safety hazards. Required postings and readily available for review were observed on the hallway. Smoke alarms and carbon monoxide detectors are interconnected also tested and function properly. There were fire extinguishers observed to be fully charged in the facility and last serviced on 08/09/2023. The last fire drill was conducted on 10/10/2023. The following was observed:

Structure: The program is housed in a single story structure on the corner of a semi-heavy traffic avenue and alley behind it, with parking lot and consists of a reception area, three (3) offices, three (3) storage rooms, five (5) program breakout rooms, locker area for client personal storage, kitchen, a total of four (4) bathrooms; two (2) designated for staff and two (2) designated for clients. There's no bodies of water on the premises. All outdoor and indoor passageways are free of obstruction.


Kitchen: The kitchen appliances and fixtures were functional. Knives, cutlery and other sharp kitchen utensils are stored and locked in the kitchen cabinet. LPA found a sufficient amount of perishable and non-perishable food at Day Program properly stored. There is one (1) commercial size refrigerator in the kitchen area. Dishes, cups and flat ware are stored in the kitchen cabinet, inspected and in good repair.

Program Breakout rooms:
The program site is clean, safe, sanitary, and in good repair for the safety and well-being of clients, employees, and visitors. All furnishings are in good repair, lighting is good, walls, ceiling and floors are also in good repair. Each client is accorded safe, healthful, and comfortable accommodations, furnishings and equipment to meet his/her needs.
Bathrooms: There are two (2) bathrooms designated for clients. All toilets and sinks are maintained in a safe, sanitary, operating condition with provisions/equipment for handicapped individuals. Hot water temperature was tested between 108 to 117 degrees Fahrenheit.
Storage rooms: All the cleaning solutions are locked and stored in the hallway closet. Disinfectants and cleaning solutions are locked and inaccessible to clients.
First-Aid Kit: LPA reviewed the first-aid kit. The first aid kit has all required supplies.
Surrounding grounds: Entry/exits were free of obstruction. The outdoor area was free of hazards. No bodies of water were observed at the facility.
Resident Files: LPA conducted a file review of resident records to insure compliance of licensing forms.
Staff Files: LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms.

The licensee ensures that each client receives first aid services. Prior to accepting a client into care, the licensee obtained and kept on file documentation of the client's medical assessment. The client's medical assessment includes the results of an examination for communicable tuberculosis and other contagious/infectious diseases.
The licensee ensures that a separate, complete, and current record is maintained for each client. Initial needs and services plan is updated as necessary, but nonetheless an Individual Personal Plan from the Regional Center is on file for each client, which serves the same purpose.


Some staff and clients were also interviewed using the CARE Tools questionnaire.

Pursuant to Title 22 Division 6 of the CA Code of Regulations, no deficiencies observed during the visit.
