Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted a Case Management Visit in reference to confirming closure of the facility.

Upon arrival LPA observed a construction crew working on the interior of the building. LPA approached the personnel that were inside the courtyard of the grounds and met with an individual who identified himself as Ernesto Martinez. Per Ernesto no one is occupying the buildings. LPA could see inside the buildings and observed that the interior of the buildings had been completely gutted and that it did not appear that anyone was or could occupy the premises at this time. LPA attempt to contact Administrator via-phone, number is no longer in service. There was no staff for LPA to met and sign the report.

LPA did confirm that the facility has ceased operation.
