Licensing Program Analyst (LPA) Tihesha Smith conducted an unannounced complaint visit to this facility to deliver findings. LPA Smith was greeted by staff and LPA Smith disclosed the purpose of the visit.

Staff locked resident in care in their room for an extended period of time.
It was alleged that Staff locked Resident #1 (R1) in their room for an extended period of time. To investigate the allegation, on 08/07/25 LPA Smith toured the facility at 10:55 am. At approximately 11:10 am, LPA Smith interviewed one (01) staff member, reviewed and requested copies of facility documents relevant to the investigation from approximately 11:20 am 12:25 p.m. On 10/20/25 LPA interviewed four (04) residents, one (01) staff from approximately 2:55 pm- 3:09pm, and on 10/30/25 interviewed one (1) staff at 3:35 pm. LPA was unable to reach the reporting party (RP) for an interview during the course of the investigation. During interviews with staff, all staff stated that they have never locked R1 or any of the residents in their room for any period of time and have not witnessed any staff locking a resident in their room. day programs from approximately 8:00 am to 2:30 pm or 3:45 pm at the latest Monday – Friday. S1 also revealed residents are out in the community on the weekends. During the physical plant tour, LPA did not observe any residents in the home or in any of the bedrooms. LPA observed the bedroom doors and locks and found no external locking device that would prevent a resident from opening the door doors from inside the bedrooms and no external locking device on the windows. During interviews with residents, all residents revealed they have not been locked in their room by staff and have not witnessed any staff locking any other residents in their rooms. R1 stated they have to go to work in the mornings, but they like to stay in their room after work to relax and watch movies.
Based on observations and interviews, there is not enough information to verify the allegation. Therefore, the allegation in UNSUBSTANTIATED at this time.

Staff left residents unsupervised while in care.

It was alleged that Staff left R1 unsupervised in the facility. To investigate the allegation, LPA Smith conducted a physical plant tour at 10:55 am on 08/07/25, interviewed staff at 11:10 am, requested and reviewed documents from 11:20 am to 12:25 pm. Additional interviews with staff and residents were conducted on 10/20/25 at 2:55 pm and 10/30/25 at 3:35 pm. Two (2) residents were unable to be interviewed due to not being present during the complaint visits. A review of the staff schedule shows facility has a minimum of (01) caregivers per shift as one caregiver is a live-in. During interviews with staff, three (03) out of five (05) staff revealed they have not left residents unsupervised and have not arrived for their shift with staff not present in the home. S1 & S3 revealed when residents are at work or at day programs the staff may not be present in the home but return before residents get off work. During interviews with residents, all resident’s stated staff are present when they are in the home and have never been left home alone by the staff. R1 stated staff would never leave residents alone in the home and staff have to be there because it’s there job to watch them.

Based on record review and interviews, there is not enough information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time.
