Facility is not allowing resident to visit privately during reasonable hours and without prior notice with visitors from outside the facility.

Facility is retaliating against resident for complaints filed on behalf of resident.

Licensing Program Analysts (LPA) Tao, conducted an unannounced 10 day complaint visit to this facility. Upon arriving at the facility, LPAs met with Administrator, Pamela Parsons. LPAs explained the purpose of today’s visit is to discuss the above mentioned allegations.
The investigation consisted of resident interviews, staff interviews, resident file review and facility’s visitor policy review.

In regards to the allegation: Facility is not allowing resident to visit privately during reasonable hours and without prior notice with visitors from outside the facility. During today's visit, LPAs interviewed the Staff and Residents. Resident, victim, did not recall details regarding not allowing visitor to visit resident privately, not having reasonable hours to visit, nor visiting resident from outside of the facility without prior notice. Three (3) out of four (4) residents stated their visitors are allowed to visit them privately and have a reasonable amount of time for visitation. Four (4) out of four (4) staff stated visitors are allowed to visit residents privately.

Staff interviews revealed that visitor was not being asked to leave the facility but he left by his choice.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

In regards to the allegation: Facility is retaliating against resident for complaints filed on behalf of resident.
Based on resident interview, Resident, victim, did not recall details regarding resident being retaliated by filing complaints. Three (3) out of four (4) residents stated they did not aware of anyone being retaliated for filing complaints. Two (2) out of four (4) staff denied any resident who was being retaliated because of filing complaints. Two (2) out of four (4) staff stated that they were not aware of any resident who was being retaliated due to filing complaints. Per the review of visitor review policy, it stated that it is preferrable to schedule appointments prior to visit. Visitor was able to visit and meet residents even no appointment was scheduled. Therefore, visitor who was unable to schedule appointment to visit residents was not related to retaliation.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

An exit interview was conducted with Administrator. A hard copy of the report were provided to Administrator.
