Licensing Program Anayst (LPA) Elena Mallett arrived at the facility for an unannounced Complaint investigation. LPA was met by staff in charge Miyeong Kim. Staff Kim contacted Licensee Eugene Choi by phone he arrived at the facility approximately 45 minutes later.
LPA requested and obtained resident and staff rosters. The investigation conisisted of interviews with Administrator, Licensee, and three staff (S1-S5) and with five residents(R1-R-5). The following documents were obtained from the Administrator's personnel file; copy of government ID, Application for Employment,Health Screening, proof ofAdministrator's RCFE certificate via Licensings website, copies of ongoing Continuating Education certificates. LPA toured the physical plant and no health and safety risks were observed.
Regarding the allegation: Facility does not have a qualified Administrator: It is alleged that the current Administator is not qualified to be an Administrator.

Licensee/Business Manager Eugene Choi and current Administrator state that Administrator Watanabe has a current Administrator's certificate and and keeps up to date with Continuing Education units. Current Administrator states that current Administrator worked for several years at other Senior Care Facilities that provided the experience needed to be lead a facility.
LPA reviewed Administrator Watanabe's Personnel file and found documentation of required experience, training and education for the job of RCFE Administrator along with Health screening, TB test. Review of Licensing databases show Hyun Joo Watanabe ( aka Jessica Shin) to have a criminal background clearance and RCFE Administrator Certificate that is from 11/19/2025-11/18/2027. Administrator's Personnel file contained a screen shot from CDSS website showing Administrator's certificate held from 11/10/23-11/18/25.
LPA observed the facility to be well maintained and LPA observed current Administrator to interact professionally with staff and residents during visit. Through interviews, record review and observation the investigation revealed that the facility has a qualified Administrator.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED. Exit interview was held with Licensee Eugene Choi and a copy of this Licensing report was provided.
