Licensing Program Analyst (LPA) Martha Arroyo conducted an unannounced case management visit today. The purpose of this visit is to conclude an investigation regarding an incident that occurred on 03/22/2023. Upon arrival, LPA met with Assistant Program Coordinator, Riley Breen and the reason for the visit was explained. Entrance Interview.

On 03/27/2023, the Department received a self-reported incident regarding Client #1 (C1). On 03/22/2023 at approximately 3:00 p.m., C1 confided in staff that they had been sexually assaulted some time last year in 2022 by two individuals also enrolled in the same day program.

During the initial visit on 04/03/2023, LPA Peraldi conducted a case management visit to follow up on the incident involving C1, conducted an interview with the Assistant Program Coordinator at 12:44 p.m., obtained copies of pertinent documents at 12:58 p.m., and conducted a physical plant tour to ensure there were no health and safety concerns at 2:07 p.m. On 03/29/2023, this incident was referred to Community Care Licensing Division (CCLD) Investigations Branch (IB) and assigned to Investigator Laura Garcia.

Information obtained from the investigator’s interviews revealed that neither C1 nor both individuals mentioned were enrolled in the Pathpoint Day Program at Simi Valley or Thousand Oaks and C1 only participated in the community-based integrated program which did not consist of C1 physically interacting with other clients. Additionally, interviews conducted revealed the day program has 1 staff per 4 clients and staff are consistently supervising the clients. Furthermore, documents obtained revealed that C1 had made similar allegations back in 2005 and 2011; however, these incidents were investigated by Simi Valley Police and deemed unfounded due to inconsistent statements and lack of evidence. Therefore, based on the information and documentation obtained and reviewed, there is insufficient evidence to support the allegation. No further follow-up is required at this time.

Exit interview conducted. No deficiencies cited at this time. A copy of the appeal rights and report was issued.
