Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted a subsequent complaint visit to the facility to investigate the above allegations. LPA met with the wellness director, Robert Erio, and was advised about the visit. At 9:15 AM, LPA conducted a physical plant tour to ensure the health and safety of the clients in care.

An entrance interview was conducted.

Allegation #1: Staff left clients in soiled diapers for an extended period of time

It was alleged that clients would remain unchanged, and clients would have an odor. To investigate the allegation on 3.11.2025, LPA conducted a physical plant tour at around 10:30 am and observed that all client attendees were clean and well-groomed. LPA did not experience any odor.

During interviews with staff, it was revealed that they check on all attendees. Staff would change the attendees as needed or requested, and do not leave residents soiled for an extended time. LPA interviewed a sample of ten (10) attendees. All ten (10) attendees expressed no concern regarding this allegation. Based on interviews and observation, there is not enough information to verify the allegation, therefore, the allegation is UNSUBSTANTIATED at this time.

Allegation #2: Staff speak inappropriately to clients in care.

It was alleged that staff #5 (S5) and staff #14 (S14) made inappropriate comments to attendees. On 3.11.2025 LPA interview staff and attendees at 10:50 AM. LPA interviewed a sample of ten (10) attendees. Interview with ten (10) attendees, and all ten (10) attendees have not witnessed or experienced staff speaking inappropriately to attendees and have not been spoken to inappropriately by staff. Clients #5 (C5) and client #10 (C10) attendees with S5 and S14, respectively, denied witnessing and experiencing staff being rude to them. Interviews from clients revealed that they are happy and have not experienced such treatment from staff. Interviews with staff revealed that they treat all their clients with respect and dignity. During the LPA's initial visit, the LPA did not observe or witness staff speaking inappropriately to attendees. LPA is unable to corroborate the allegation. Based on interviews and observation, there is not enough information to verify the allegation, therefore, the allegation is UNSUBSTANTIATED at this time.

No health and safety hazards were observed during the visit.

An exit interview was conducted, and a copy of the report was issued.
