Licensing Program Analyst (LPA) Tuesday Cabiness conducted an unannounced annual required visit.
LPA met with Executive Officer Chris Bratzel, who was explained the purpose of the visit. Since last year's annual inspection, there has been no changes. The Activity Center is still not in use, but they are keeping the license for potential for program.The facility is still not vendored by North Los Angeles Regional Center (NLARC).

A tour of the physical plant was conducted including the surrounding grounds. There is one entrance being utilized at the facility. There is a screening area is located immediately upon the entrance. Sign in sheet, infrared thermometer, hand sanitizer, gloves and masks will be available. LPA entered the facility with the Executive Officer. LPA observed tables, chairs, and activity equipment for clients to use. There are computers that are available The facility has enough PPE supplies. The facility has a large common area and is located adjacent to the community kitchen distribution center. The facility has three (03) bathrooms. Bathroom #1 is located inside the center, is a unisex bathroom, is handicapped accessible. Bathroom #2, which is located outside, is designated for women only, is handicap accessible. Bathroom #3 is designated for men only, is handicap accessible. All bathrooms were checked for proper operations and cleanliness. Fire extinguisher is fully charged. There are dual smoke/carbon monoxide detectors installed at the facility. There is a TV set in the facility and there are lockers inside the facility for clients' use. The facility is equipped with fire sprinkler system. The facility also has storage area with refrigerators and freezers, which was stocked with non-perishable and perishable food.

No health and safety hazards observed at the time of the visit. Exit Interview Conducted. Copy of this report issued.
