25 | At 9:45 a.m. on 03/27/2025, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced annual inspection. LPA met with the administrator and disclosed the reason for the visit.
A file review was conducted prior to today’s visit.
The facility was last visited on 01/15/2025 for a complaint visit. It is a single story building with six (06) bedrooms, two (02) bathrooms, kitchen, laundry room, common areas, and outdoor areas. It has an approved fire clearance for six (06) ambulatory residents.
LPA observed a maintained front yard and patio furniture in good repair. Postings at the front and in the kitchen included the facility license, confidential complaints contacts, emergency disaster plan, house rules, neighborhood grievance procedure, non-discrimination notice, personal rights, weekly menu, activity calendar, and the facility sketch.
The living room contained a television, karaoke, reading material, exercise equipment, games, puzzles, and furniture in good repair.
The facility has six (06) bedrooms. Two (02) bedrooms are designated as staff rooms. The staff rooms were locked and free of hazards. All bedrooms contained a chair, lamp, nightstand, storage, and a bed with adequate bedding. All furnishings were clean and in good condition.
The facility has two (02) bathrooms. One (01) bathroom is shared. All bathrooms contained liquid soap, paper towels, handwashing instruction sign, trash can with a tight fitting lid, grab bars near the toilet and shower, and a non-skid mat in the shower.
