Licensing Program Analyst (LPA) Michael Cava conducted a subsequent visit to the facility to close out the investigation regarding the above facility. During the course of the investigation, interviews and physical plant inspection were made. LPA met with the administrator, Carolina Garcia-Trejo, and advised her of the visit.

Staff are making inappropriate comments towards residents:
In regards to the allegation, it was reported that a staff had been telling residents that are enrolled in the Assisted Living Waiver (ALW) Program that they shouldn’t be admitted into the facility. At the time of the initial investigation, the facility is retaining approximately 69 residents that are on the ALW program. Random residents on this program were interviewed, and denied this comment was ever made to them. Staff were also interviewed, and they weren’t aware that this comment was made to them, as they’ve never received any complaints or concern from the residents. wasn’t enough evidence to corroborate the allegation. Therefore, the allegation is deemed Unsubstantiated at this time.

Facility is not providing adequate food service for residents:
In regards to the allegation, it was reported that facility has false advertisement regarding their food. Residents were interviewed regarding this allegation, and expressed no complaints or concerns regarding the food service. LPA conducted an inspection of the kitchen to insure an adequate supply of perishable and non-perishable food, and that the food storage is maintained and the food is of good quality and not expired. Menus were also reviewed. Based on the information obtained, there wasn’t enough evidence to prove that the facility is not providing adequate food service. Therefore, the allegation is deemed Unsubstantiated at this time.

Staff denies residents food:
In regards to the allegation, it was reported that staff have been acting as a “food police” towards residents that are diabetic, or require special diet. Staff was seen taking food off these resident’s plates if they think it was too much. Interviews made with the administrator and staff who deny the allegation. Food is never withheld or taken from residents whether or not they require a special diet. For residents with a special diet, kitchen staff are made aware of their food restrictions and make sure their dietary plan is followed. Residents were interviewed and they also denied food ever being taken from them from staff, or being denied food service. Based on the information obtained, there wasn’t enough evidence to prove the allegation of staff denying residents food. Therefore, the allegation is deemed Unsubstantiated at this time.
