Licensing Program Analyst (LPA) Quoc Huynh arrived at the facility unannounced for a required one-year visit. LPA arrived at 12:00PM and met with the Administrator Diana Gevorgyan. Entrance interview conducted.

Beginning at 12:20PM, the LPA and Administrator conducted a tour of the physical plant to ensure residents’ health and safety, and the facility is in compliance with Title 22 regulations. The following was observed:

KITCHEN: Knives and cleaning supplies were stored inaccessible in locked cabinets and drawers. Kitchen appliances and fixtures were clean and functional. The facility also had a sufficient supply of perishable food. Food in the refrigerator and freezer were of good quality, however two (2) food items had expiration dates of 07/12/2025 and 04/23/2026. The Administrator discarded the expired food. On 04/11/2025, LPA Huynh’s annual visit noted broken drawers in the refrigerator that the Licensee agreed to fix. During today’s visit, the drawers remained broken.

COMMON AREAS: The common areas included a living room, den, and dining room. All furniture was clean and in good condition and a fireplace was observed to be properly screened. There was sufficient space to accommodate both indoor and outdoor activities. Nightlights were observed throughout the hallways. Emergency food was observed by the entrance and emergency water was located in the den. The facility had a laundry room that contained general storage and supplies, locked chemicals, machines in good condition, and a sufficient supply of non-perishables.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

BEDROOMS/RESTROOMS: There were five (5) total bedrooms: three (3) resident rooms and two (2) staff rooms. Bedrooms were observed to have appropriate furniture, linens, and sufficient lighting. There were no visible hazards or inconsistencies observed. The LPA observed that one (1) staff room was not secured and contained accessible chemicals. The staff obtained their key from a dresser and secured the room during the visit. Extra linens were stored in the hallway closet. There were two (2) total restrooms: one (1) private resident restroom and one (1) shared common area restroom. All bathrooms were observed to be clean and sanitary, supplied with appropriate paper and hygiene products, and had hand washing signs. Hot water temperature was tested and measured at 117.7 degrees F.

OUTDOOR AREA: The facility’s backyard had an in-ground pool that was secured by a fence and locked. Exterior passageways were clean and clear of any obstructions. There was one (1) side gate for emergencies and an additional exit in the rear that led to an alley. Three (3) locked sheds were observed that contained general storage.

RECORD REVIEW: Record review began at 12:55PM. Resident records were reviewed for, but not limited to care plans, physician's report, admissions agreement, and consent forms. Resident files reviewed contained all required documents. Personnel records were reviewed for, but not limited to health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All staff records reviewed were in compliance at the time of the visit.

INFECTION CONTROL/EMERGENCY DISASTER PLANNING: During today’s visit, the LPA reviewed the facility's infection control plan as well as the facility's emergency disaster plan. Both documents were observed to be complete and reviewed/updated annually. The facility conducts emergency drills as required, with the last drill documented on 02/07/2026. Fire extinguishers were observed throughout the facility and were purchased on 03/05/2026. Smoke and carbon monoxide detectors were tested at 1:58PM and were operational.

MEDICATION REVIEW: LPA reviewed two (2) resident medications at 2:02PM. Resident medications were centrally stored and inaccessible to residents in care in a hallway cabinet. Medications were checked for labels, expiration dates, and were properly documented on the centrally stored medication and destruction record.

Pursuant to Title 22 CA Code of Regulations and/or the Health and Safety Code, the following deficiencies were cited (Refer to LIC 809-D).

Exit interview conducted. A copy of the appeal rights and today’s report was reviewed and provided.
