At 11:35 a.m. on 09/28/2022, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced annual visit. LPA met with the Business Coordinator and disclosed the reason for the visit. LPA toured the facility inside and out.

The facility was last visited on 05/24/2022 for a case management visit. It is a two story building with private and shared bedrooms, private and public bathrooms, kitchen, dining room, libraries, hair salon, common areas, and outdoor areas. It has an approved fire clearance for 121 nonambulatory residents, of which 27 may be bedridden. The facility serves residents in assisted living and memory care settings. Approved hospice waivers for 20.

A sign was posted at the front regarding the facility’s visitation and masking policies. LPA was screened for infectious disease upon entry. The screening station contained digital thermometer, surgical masks, and hand sanitizer. Postings at the front included confidential complaint contacts, Ombudsman contacts, emergency contacts, resident rights, rights of resident councils, facility license, and Emergency Disaster Plan. LPA also saw daily and monthly activity schedules and a daily menu in the dining room. Residents sat, conversed, and ate lunch in the dining room. The kitchen was sanitary, and all employees were wearing masks. At 12:02 p.m. LPA a fully charged fire extinguisher between the kitchen and dining room. It was last inspected on 07/22/2022. Other fire extinguishers were located throughout the hallways, and all were accompanied by emergency lighting. Walls, floors, ceilings, windows, screens, and blinds were clean and in good repair. At 12:05 p.m LPA measured the room temperature to be 74 degrees Fahrenheit. A fireplace at the front was appropriately covered. Libraries were located in the middle of the hallways on the first and second floors. Books, television, and activity supplies were available.

LPA and Wellness Coordinator toured Bedroom #220. It contained a chair, nightstand, lamp, storage, and bed with adequate bedding. All furnishings were clean and in good condition. Bedrooms and private bathrooms contained a pull-cord system to call staff. The bathroom contained liquid soap, paper towels, trash can with a tight fitting lid, grab bars near the toilet and shower, and a non-skid mat in the shower. At 12:12 p.m. LPA measured the water temperature in the bathroom to be 108.9 degrees Fahrenheit. LPA inspected a public bathroom as well. At 11:59 a.m. the water temperature measured at 112.6 degrees Fahrenheit. LPA observed liquid soap, paper towels, a handwashing instruction sign, and a trash can with a tight-fitting lid.

All emergency exit paths and stairwells were free from obstructions. Exit doors used delayed egress and were unlocked. Outdoor areas contained shaded furniture and gardening areas. At approximately 12:35 p.m. LPA tested the carbon monoxide detector to be operational. LPA observed the most recent fire system tests, and all systems passed.

From an interview with the Wellness Coordinator and Business Coordinator, the facility has improved efforts to manage resident refusals. A therapist now comes on a weekly basis, and a psychiatrist is available on a monthly basis. Additional staff training was provided to address resident refusals and skin integrity, and both coordinators reported fewer instances of refusals and integrity issues.

During today's inspection, the facility is in compliance with Title 22 regulations. No citations issued.
