25 | At 11:35 a.m. on 09/28/2022, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced annual visit. LPA met with the Business Coordinator and disclosed the reason for the visit. LPA toured the facility inside and out.
The facility was last visited on 05/24/2022 for a case management visit. It is a two story building with private and shared bedrooms, private and public bathrooms, kitchen, dining room, libraries, hair salon, common areas, and outdoor areas. It has an approved fire clearance for 121 nonambulatory residents, of which 27 may be bedridden. The facility serves residents in assisted living and memory care settings. Approved hospice waivers for 20.
A sign was posted at the front regarding the facility’s visitation and masking policies. LPA was screened for infectious disease upon entry. The screening station contained digital thermometer, surgical masks, and hand sanitizer. Postings at the front included confidential complaint contacts, Ombudsman contacts, emergency contacts, resident rights, rights of resident councils, facility license, and Emergency Disaster Plan. LPA also saw daily and monthly activity schedules and a daily menu in the dining room. Residents sat, conversed, and ate lunch in the dining room. The kitchen was sanitary, and all employees were wearing masks. At 12:02 p.m. LPA a fully charged fire extinguisher between the kitchen and dining room. It was last inspected on 07/22/2022. Other fire extinguishers were located throughout the hallways, and all were accompanied by emergency lighting. Walls, floors, ceilings, windows, screens, and blinds were clean and in good repair. At 12:05 p.m LPA measured the room temperature to be 74 degrees Fahrenheit. A fireplace at the front was appropriately covered. Libraries were located in the middle of the hallways on the first and second floors. Books, television, and activity supplies were available.
