Licensing Program Analyst (LPA) Vasallo initiated a subsequent complaint investigation for the allegations listed above. Due to the situation surrounding the Coronavirus Disease 2019 (COVID-19), and to implement mitigation measures, today’s complaint investigation was conducted telephonically with Leticia Flores, Assistant Administrator. The initial tele-visit was conducted on 5/5/21.

The investigation consisted of the following: LPA obtained a resident and staff roster. 5 residents were interviewed during the initial tele-visit. An additional resident was interviewed during the subsequent visit and 6 staff were interviewed.

The investigation revealed the following: Staff interviewed denied the allegations. Staff indicated they have never witnessed staff yell at residents or call residents inappropriate names.

Residents were randomly chosen for interview. Interviews conducted with residents did not corroborate the allegations. Residents reported that they do not know of or have witnessed staff yell or call residents names. The complaint did not specify any witnesses to the allegations.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

Exit interview held with Leticia Flores and Marina Pink and a copy of the report was emailed for signature.
