25 | Licensing Program Analyst (LPA) Zabel Chochian conducted a Required - 1 year visit and met with Yaniv Geller and Yanett Perez. Reason for the visit was explained.
LPA and Ms. Perez toured the physical plant areas at approximately 11am. The facility consists of single-floor divided into various work areas including: Relaxation room, Computer room, two (2) class room areas, which in one of the class rooms there is a small sink, cabinets, microwave and refrigerator. Consumers bring there own lunch and facility provides snacks and water. Facility has two (2) restrooms. Water temperature in the restrooms was measured at 113.9 degrees and fixtures observed operable. At this time the program is not storing or administering any medications to current participants. Facility smoke detectors are hardwired. Facility provided record of last inspection conducted by Absolute Fire Company which was on 6/23/2023. Carbon Monoxide detector tested was operable during visit. Fire extinguishers observed serviced 08/08/2023, disaster drills conducted once a month (last fire drill conducted 4/2024 and earthquake drill completed 5/16/2024); first aid kit observed with guide and all required items. Storage room observed locked during visit with cleaning supplies and other toxins.
LPA Chochian also reviewed facility files at approximately 11:30am and verified current staff fingerprint cleared/associated to the facility, also the staff in-service training reviewed. Approximately 12pm, Client files were reviewed. Two out of four client files reviewed did not have a current IPP on file. Mr Geller provide proof that IPP was requested for client files from Regional Center. A technical violation was issued during todays visit. Mr. Geller will follow up with Regional Center to ensure client IPPs are submitted timely.
