On 11/01/22 Licensing Program Analysts (LPAs) Joscelyn Martinez and Tuesday Cabiness arrived at the facility to conduct a subsequent unannounced complaint investigation. Upon arrival LPA met with staff and later met with administrator Angela Apoyan. The purpose of the visit was explained.

Allegation: Staff did not safeguard resident's personal belongings.

It is alleged that the resident's personal belongings are not being safeguarded by the staff. To investigate this allegation LPAs conducted interviews with six (6) residents. Five (5) out (6) residents stated they do not have any issues with their belongings not being safeguarded by the staff. Interviews with staff and administrator stated if resident reports a missing or damaged item facility will document the item, search for the item and if not found or the item is damaged by staff, facility will replace the item. Based on interviews this allegation is deemed Unsubstantiated.

Allegation: Staff are not following resident's dietary orders.

It is alleged that staff is not following resident's dietary orders. To investigate this allegation LPA conducted interviews with residents, staff, and collected relevant document pertaining to the allegation. On 11/01/22 LPA conducted a tour of the kitchen and observed sufficient 7-day non-perishable and 2-day perishable items. Items observed were of different variety and of nutritional value. Interviews with residents revealed facility is accommodating to their special diets. During today's visit LPA observed caregiver preparing hamburgers with a side salad for lunch. On 02/22/22 LPA observed caregiver preparing fresh fruits for resident's snack time. Interviews with staff revealed they follow dietary order for residents such as providing low sodium meals or diabetic friendly meals. Based on interviews and observation, this allegation is deemed Unsubstantiated.

Allegation: Resident is not accorded privacy.

It is alleged that staff are not proving residents privacy in their rooms. To investigate this allegation LPAs conducted interviews with six (6) residents and two (2) staff. Interviews with residents revealed that five (5) out of six (6) residents are accorded their privacy and staff will knock on their door prior to entry. Interviews with staff revealed staff are trained to knock before entering a resident room and accord them with privacy when requested by the residents. Based on interviews this allegation is deemed Unsubstantiated.

Allegation: Licensee did not provide a comfortable environment for residents

It is alleged that the licensee is not providing residents with a comfortable environment in the home. To investigate this allegation LPA conducted interviews with six (6) residents and two (2) staff. Interviews revealed that all six (6) residents feel comfortable in the home. Interviews with staff reveled they have not had any residents complain that they feel uncomfortable living in the home. Based on interviews this allegation is deemed Unsubstantiated.
