Licensing Program Analyst (LPA) Jose Tan conducted an unannounced subsequent visit to this facility to further investigate the above allegations. LPA met with Executive Director Peter Bonilla and explained the reason for the visit.

LPA conducted physical plant tour at 9:23 AM, requested facility documents relevant to the investigation at 10:12 AM, reviewed records between 10:30 AM to 11:30 AM and interviewed staff and residents between 11:30 AM to 1:30 PM. Regarding the allegation that Staff are not properly trained, it was alleged that Staff #1 (S1) did receive First Aid/CPR training or any general training upon being hired. LPA's record review between 10:30 AM to 11:30 AM today revealed that S1 was provided training as required prior to S1's start date and continued on S1's first week of employment. LPA also reviewed four (4) random staff records and revealed that all staff records reviewed had all the required training on record.

Regarding the allegation that Staff are not able to effectively communicate with residents in care, it was alleged that Staff are Spanish speaking and do not communicate with residents in English. LPA's interview with staff revealed that S1 speaks Spanish but could communicate in English. LPA's interview with three (3) staff on duty at memory care today between 11:30 AM to 1:30 PM revealed that all three (3) staff interviewed communicates in English well and answered all LPA's question.

Regarding the allegation that Staff handled resident in care in a rough manner, it was alleged that staff members handled Resident #1 (R1) roughly. LPA attempted to interview R1 but R1 was incoherent and unable to answer any of the LPA's question. LPA's interview with three (3) staff today between 11:30 AM to 2:30 PM revealed that all three of them did not witness any other staff handling R1 in a rough manner. LPA's interview with the Wellness Director today also revealed that the Wellness Director did not receive any report of any resident in Memory Care being handled roughly by any staff in Memory Care.

Based on the information gathered during this and prior visit, these allegations are deemed unsubstantiated at this time.
