At 12:00 p.m. on 09/20/2023, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced case management visit. LPA met with the Administrator and disclosed the reason for the visit.

Today’s case management visit was a subsequent visit to follow up on an incident previously reported by the facility in which Staff #1 (S1) made an unwanted sexual advance towards Resident #1 (R1).

LPA interviewed R1 today at 12:00 p.m., a visitor at 12:30 p.m., and the Administrator at 2:30 p.m.

The Administrator reported the incident appropriately and placed S1 on administrative leave until further notice.
