ALLEGATION: Staff retaliated against complainant
It was alleged that that staff was wrongfully terminated in retaliation for their complaints filed against facility. Interviews of administrative personnel, including the ED revealed that S1 did not follow specific instructions to perform their job duties. ED and S1’s immediate supervisor spoke with S1 many times. Over time, S1’s insubordination led to disciplinary actions taken against S1 and subsequently S1 was terminated. A review of S1’s file verified the information revealed by administrative personnel.
Based on interviews and record review there is insufficient information to verify the allegations. Therefore, the allegation is UNSUBSTANTIATED at this time.

ALLEGATION: Administrator is not able to provide adequate attention to the management and administration of the facility.

It was alleged that the facility’s ongoing issues are directly related to the leadership of former ED, who has exhibited a pattern of dishonesty and unprofessional behavior, which has contributed significantly to the facility operation. Critical staff positions remain unfilled, including payroll, Wellness, nursing, memory care, and maintenance. These vacancies contributed to the facility's overall dysfunction. LPA's interviews with administrative personnel, including the current ED, RSD, Concierge, & Business Office Manager (BOM) revealed that facility is going through some changes. The Former ED is no longer in charge. However, the facility operation was not reflected as they always have other leads relieving ED. During the investigation, LPA Alvizar-Ettima and LPA Comer were unable to obtain the information regarding former ED's separation from the company. A review of former ED's file did not reveal any information to support the allegation. Overall investigation did not reveal any sufficient information to support the allegation.
Based on interviews and record review, it was concluded that although the allegation may have occurred, there is no measurable and verifiable information to confirm validity of the complaint. Hence the allegation is UNSUBSTANTIATED at this time.

No immediate health and safety issues were noted at the time of this visit. Exit interview was conducted and a copy of report was issued.
