25 | Licensing Program Analyst (LPA) Ashley Smith arrived at the facility unannounced to conduct a Case Management - Other visit, with the purposes of conducting a health and safety check and a file audit. At the time of the visit, there were two (2) staff to six (6) residents. The LPA spoke with Administrator Robin Aquino over the phone, whom was unable to be present. The LPA informed them of the reason for the visit.
The LPA conducted a physical plant tour at 9:06 a.m. There were no immediate health and safety concerns noted during the visit. The LPA conducted a file audit from 9:15 a.m. - 10:15 a.m.
The LPA compared the personnel report to the Department clearance list and identified that one out of two staff (S1) working today are without fingerprint clearance. The LPA spoke with Ms. Aquino about this, whom confirmed that they have continued to work through this, as there were challenges in having S1 associated. As this is the first violation within the past twelve (12) months, a civil penalty shall be assessed for a maximum of five days, at $100 a day, per person.
Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D). Exit interview conducted and report issued. Civil penalties assessed. Ms. Aquino authorized staff to sign the report. |
