Licensing Program Analyst (LPA) Tuesday Cabiness met with Licensee Paruyr Mkrtchyan to conduct a physical plant inspection, due to the facility ceasing operation and no longer has residents. LPA entered the facility and observed it to be in construction and remolding. There were no residents and Licensee informed LPA that at the time of closure there were only (2) residents, and one passed away, and the other resident moved with family.

LPA obtained the original license and will close facility in LIS.
