Regarding the allegation: Staff does not ensure the facility is free of pests. It was alleged that there are ants and roaches in a second floor apartment and hallway.
Regarding the allegation: Staff does not ensure the facility is free of rodents. It was alleged that a rat was observed in the kitchen.

To investigate the allegations, between 11:20am – 1:30pm, LPA conducted interviews with the Executive Director, twelve (12) staff, and four (10) residents. All resident interviewed denied seeing any insects, rats or droppings in their rooms or at the facility. Of the twelve (12) staff interviewed, three (3) mentioned that they were aware that a pest control company has come to treat but no evidence of roaches, ants or rats were found. Nine (9) staff had no knowledge and have not seen any dead or alive insects, rats or droppings. LPA interviewed the Executive Director who stated in April 2026 and August 2026, someone had contacted County of Los Angeles Public Health Department to report cockroaches and rodents in the kitchen. Inspectors from County of Los Angeles Public Health Department inspected the facility on 04/20/2026 and 08/05/2026 but no evidence of live or dead cockroaches were observed. In addition there were no evidence of rodent intrusion, gnawing marks, droppings were observed. Executive Director stated facility uses a professional pest control company called Ecolab Pest Elim, on a weekly basis, as a preventive measure. The pest control company has treated multiple areas but has not reported any infestation. Based on interviews and record review, it was revealed that the facility uses the pest control company as general maintenance and they have taken all measures to ensure there is no pest infestation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is UNSUBSTANTIATED.

Exit interview was conducted. Copy of report provided.
