Due to the destruction of Grandeur Adult Residential by fire, the associated complaint report was delivered to Ventura Adult Residential, a sister facility under the same ownership by Licensing Program Analyst (LPA) Tihesha Smith at 9:30 am. LPA Smith met with Administrator Cindy Garcia and disclosed the purpose of the visit.
On 02/27/25 due to circumstances surrounding Eaton Fire-Disaster, LPA conducted an initial virtual visit. LPA met with the Co-Administrators and gathered information pertinent to allegation. On 06/03/2025, LPA Alvizar-Ettima conducted collateral at Airbnb home located 847 N. Rimhurst Ave. Covina, CA 91724. At 9:30am LPA met and spoke with DSP staff present at the Airbnb and at 9:45am contacted Co- Administrator Cindy Garcia via telephone. In addition, at 10:00am LPA was able to speak with a client #1 (C1).
Due to facility destruction, LPA was unable to gather C1 records. However, prior to this visit on 07/10/25 LPA Antonia Alvizar-Ettima was able to review copies of C1’s records previously gathered from the facility.

to physically harm Client #1(C1), after C1 tried to speak with the police that was at the property for another incident. S1 and C1 had verbal altercation.

LPA interviewed C1verified that they had verbal altercation but was unable to provide details. LPA interviewed S1 via telephone and they denied the allegation. The information provided by S1 was consistent with accounts from other staff members. The Co- Administrator and staff interviews revealed that they did not make verbal threats to physically harm C1. Staff reported that C1 consistently displays threatening behavior toward staff members. During the investigation, LPA did not observe staff interacting with clients in an inappropriate manner.


Based on observation, interviews and documents review there is insufficient information to support the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time.
Staff are not providing adequate food service

It was alleged that on 02/20/25 C1 woke up late and requested staff #2, (S2) to make food and S2 declined to make C1 food. During the interview with C1 they indicate that there is always food but sometimes they don’t want to eat it. C1 sometimes wants different food to be made, and staff refuse. Staff interviews reveal that C1 complaints about the food but still eats it. Co – Administrator indicated that have not received any complaints about inadequate food service from other clients in care. The facility provides alternative food if clients do not like what is being served for the day. Co- Administrator indicated that staff are always making sure the food menu is created with clients’ favorites and are open for client’s suggestions. LPA interview via phone and denied the allegation. S2 indicated that C1 always wakes up late and C1’s food is always saved when they wake up. All clients received the same food as indicated on the menu. Client interviews reveal that they did not have any concerns about the food service. Overall, the food is good. Client’s interviews confirm that the facility has alternative food which is available per clients’ request. A review of the facility’s main menu and alternative options supported the information provided by the staff.

Based on observation, interviews and documents review there is insufficient information to support the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time.

No health and safety hazards were noted during this visit.
