13 | Licensing Program Analyst (LPA) Gary Tan conducted an unannounced initial complaint visit at this facility to investigate the above allegation. LPA met with Staff Erika Stinson who called the administrator Rod Stinson and explained the reason for the visit. The administrator designated Ms. Erica Stinson to sign the report.
LPA conducted a physical plant tour at 11:05 AM, requested copies of facility documents relevant to the investigation at 11:19 AM and interviewed the staff and residents between 11:30 AM to 1:30 PM. Regarding the allegation that staff are not providing adequate food service for residents, it was alleged that the pantry was basically empty and that staff are not cooking culturally diverse food. LPA's observation during visit on 06/05/25 and today at around 12:05 PM revealed that the facility has sufficient food in stock for perishable foods for two (2) days and seven (7) days for non-perishable food based on the current census. LPA's review of facility menu also revealed that the facility is serving culturally diverse food. LPA's interview with four (4) residents or more than 20% of current census revealed that four (4) out of four (4) residents get an adequate food service and culturally diverse food from the facility.
