On 12/08/25 at 01:30 pm, Licensing Program Analyst (LPA) Lorena Casillas conducted an unannounced 10-day complaint visit to the facility to investigate the above allegations. LPA was greeted and granted access to the facility by staff. LPA met with Administrator Priscilla Isordia, LPA explained the reason for the visit and an entrance interview was conducted.

At approximately 01:45 pm, LPA requested copies of client and staff rosters. LPA also requested copies of staff duties, anything related to maintenance or service calls and any documents relevant to the investigation. At 02:30 pm, LPA conducted a physical plant tour to ensure the health and safety of the clients are protected. At approximately 03:00 pm LPA conducted a file review of documents provided. Between 01:30 pm and 04:00 pm, LPA conducted interviews with Administrator, six (6) staff and four (4) out of four (4) clients.

Allegation: Staff are not cleaning client’s room.

It is alleged that staff are not cleaning client’s room. Regarding this allegation it is reported that staff are no longer entering Client #1’s (C1’s) to clean it. Interview with C1 revealed that staff clean C1’s room and the facility all the time. Interview with two (2) clients via telephone revealed that they have no concerns about their rooms being cleaned, and that staff are constantly making sure that their rooms are clean. Interview with one (1) client, also present revealed that staff are always prompting them to clean their room, but that staff do help with keeping it clean. The Administrator denied the allegation stating that cleaning client rooms and maintaining the facility clean is a main duty of all staff on all shifts. Interview with six (6) staff present in the facility, to include one on one staff, corroborated what the Administrator stated. Staff confirmed that a major part of their duties are to make sure client rooms, and the facility are maintained clean, and either assist or do client laundry. LPA toured the facility, and it was noted to be clean and free of clutter or smells. LPA inspected client rooms and noted that the rooms were clean and organized. LPA reviewed the staff duty statement where it is noted that staff is “responsible for helping residents to keep their personal property clean and neat”. LPA also reviewed staff forty (40) hour orientation where “Housekeeping and sanitation principles” is included as part of their training. Therefore, based on interviews, observations and record reviews this allegation is deemed unsubstantiated at this time.

Allegation: Staff did not ensure facility was free from pests.

It is alleged that staff did not ensure facility was free from pests. Regarding this allegation it is reported that the facility has bed bugs. Interview with Administrator revealed that they have not been made aware by either clients or staff that there are bed bugs in the facility, however an exterminator company arrived on 12/8/25 at 10:00 am to inspect the property. This was done as a preventative measure as another facility did have an infestation. The exterminator was not able to locate any bed bugs or any other insects within the facility. Administrator also stated that the clients have not shown signs of being bitten by any insects. Interview with four (4) out of four (4) clients revealed that they have not been bitten by any bugs nor have they seen any insects in their rooms or throughout the facility.

Interview with six (6) staff revealed that they have not observed any bed bugs in the facility and that clients have not reported any bites. Staff also stated that they assist clients with some grooming, and they have not observed any bites on the clients. LPA and Administrator toured the facility and inspected client bedding but could not locate any bed bugs or any other insects. LPA and Administrator also inspected living room furniture and did not observe any bugs or any other insects. Therefore, based on interviews, file review and observations this allegation is deemed unsubstantiated at this time.

Allegation: Staff are not ensuring clients’ clothes/bedding are washed.

It is alleged that staff are not ensuring clients’ clothes/bedding are washed. Regarding this allegation it is reported that Client #1’s (C1’s) clothes and bedding are not being washed. LPA interviewed C1 and it was stated that their clothes and bedding are washed, denying this allegation. LPA interviewed two (2) other clients via telephone who revealed that the staff wash their clothes and bedding. One (1) other client present at the facility also denied the allegation. Clients also stated that staff help put their things away once they are cleaned. LPA interviewed the Administrator who denied this allegation stating that helping clients with doing their laundry is a duty assigned to staff on all shifts as needed. Administrator stated that clients are encouraged to do their own laundry under staff supervision, however when clients do not show interest in this, the staff take over the duty and provide clients with clean clothes and clean linens. Interview with six (6) staff present in the facility corroborated what the Administrator stated. Staff confirmed that part of their duties are to make sure client rooms are clean to include doing their laundry. LPA toured the facility and observed client dressers and closets with clean and folded laundry. LPA also observed the linen closet to have plenty of clean linens available to the clients. LPA observed that there was no accumulation of dirty laundry, and that washer and dryer were operable. LPA reviewed staff duty statement and noted that part of staff duties are to do laundry as needed. LPA also reviewed staff forty (40) hour orientation where “Housekeeping and sanitation principles” is included as part of their training. Therefore, based on interviews, observations and record reviews this allegation is deemed unsubstantiated at this time.
