At 9:30 AM, Licensing Program Analyst (LPA) Huma Rahimi conducted an unannounced initial complaint visit. LPA met with staff, Patrick Mugenyi, and Administrator was contacted via telephone. The Administrator arrived at 10:00 AM. LPA explained the reason for the visit. At 12:30 PM, Administrator had to leave and designated Patrick Mugenyi to sign today's report.

During course of the investigation, interviews and record review were made. At 10:05 AM, LPA requested client and staff roster. At 10:10 AM, LPA requested copies of pertinent information which include, but not limited to Staff training, Training Materials, and etc., relevant to the investigation. At approximately 10:15 AM, LPA conducted a physical plant tour. Between 10:30 AM – 11:30 AM, LPA conducted an interview with the Administrator and one (1) staff.

Staff did not have required training's.
It was reported that during the annual visit conducted on 03/19/2025, by North Los Angeles Regional Center (NLARC) that three (3) staff at the facility did not have any prior experience/training working with the clients. It was also reported that Staff #1 (S1) only completed three (3) Continuing Education Units (CEUs), Staff #2 (S2) completed four (4) CEUs, and Staff #3 (S3) completed eight (8) CEUs within six (6) months of their anniversary hire dates. To investigate this allegation LPA conducted an interview with the Administrator who denied the allegation and informed LPA that the facility did provide sufficient CEUs to all staff, particularly, to S1, S2, and S3. Moreover, the Administrator informed LPA that the Administrator always utilizes credible training materials through Department of Developmental Services (DDS), Smart Seminars Videos, Title 22 Regulations, Title 17 Regulations, and facility's plan of operation. Furthermore, LPA conducted a file review of three (3) out of five (5) staff and observed that S1 was hired on 04/03/2024 and completed 45 CEUs between 04/03/2024 through 04/10/2024. S2 was hired on 05/31/2024 and completed 45 CEUs between 06/05/2024 through 06/13/2024. S3 was hired on 05/31/2024 and completed 45 CEUs between 05/31/2024 through 06/05/2024. On 04/28/2025, LPA reviewed facility's plan of operation and observed the following: "If new staff have less than 6 months experience providing direct supervision and special services, they will complete an additional 12 hours of Continuing Education within the first 6 months of being hired". Review of records revealed that S1, S2, and S3 completed 24 hours of CEUs and an additional 21 hours within the time frame, which meets the requirement. Based on interviews, record review, and facility's plan of operation this allegation is deemed Unsubstantiated at this time.
