Licensing Program Analyst (LPA) MIchael Cava conducted a complaint visit to the facility to investigate the above allegations. LPA met with the executive director, Cynthia Drachenberg, and advised her of the visit. During the course of the investigation, a physical plant inspection and interviews were made. Copies of facility records were obtained and reviewed.

Facility is understaffed:
Regarding the allegation, it was reported that there is insufficient staff to assist with resident showers, do laundry and conduct activities with residents. Interviews and a physical plant inspection were made between 9:30am-12:30pm. Ten of ten residents were interviewed and expressed no concerns regarding facility being understaffed. During the physical plant inspection, LPA observed a sufficient amount of staff. Five of five staff interviewed and deny the allegation of facility being understaffed. Between 12:30pm and 2:00pm, copies of facility records were requested and reveiwed. Per record review, LPA observed

six caregivers/med techs scheduled to work on the am shift, thirteen caregivers/med techs scheduled to work during the pm shift, and nine caregivers/med techs scheduled to work noc shift. Furthermore, LPA obtained a copy of resident shower schedule to insure staff assistance. Based on the information obtained, there was insufficient evidence to corroborate the allegation of facility being understaffed. Therefore, the allegation is Unsubstantiated at this time.

Facility restrooms are not equipped with proper hygiene supplies:
In regards to the allegation, it was reported that the facility public bathrooms are not equipped with toilet paper and paper towels. Staff would have to go to the store next door to the facility to use the restroom. It was also reported that residents would have to pay extra fees in order to obtain toilet paper. Interviews and a physical plant inspection were made between 9:30am-12:30pm. Public bathrooms were checked on the first and second floors. A supply of toilet paper and paper towels were observed. Bathrooms in resident rooms observed, and a supply of toilet paper furnished. In between that time, five of five staff interviewed. Staff denied having to ever go use the public bathroom at the store next door. Staff also denied that the public bathrooms are not furnished with toilet paper and paper towels In addition, ten of ten residents interviewed and they all denied having to pay extra fees for toilet paper as it is furnished for them and replaced during housekeeping. Based on the information obtained, there was insufficient evidence to corroborate the allegation of facility restrooms not being equipped with proper hygiene supplies. Therefore, the allegation is Unsubstantiated at this time.

Staff are not following personal hygiene practices when preparing food:
In regards to the allegation, it was reported that staff are not properly washing their hands and not using gloves when preparing food. Interviews and a physical plant inspection were made between 9:30am and 12:30pm. During the physical plant inspection, LPA observed all three kitchen staff wearing gloves while preparing meals. Three of the three staff confirmed personal hygiene practices are made during food preparation. LPA also observed postings and signs in the kitchen reminding staff when preparing meals to sanitize and wash their hands. Based on LPAs observation, it cannot be proven that staff are not following personal hygiene practices when preparing food. Therefore, the allegation is deemed Unsubstantiated at this time.

Facility is charging residents for general use hygiene supplies:
In regards to the allegation, it was reported that residents have to pay extra fees in order to obtain toilet paper and paper towel. Interviews and a physical plant inspection were made between 9:30am and 12:30pm. Ten of ten residents interviewed and denied ever being charged for hygiene supplies. Three of three staff interviewed and stated resident bathrooms are stocked with toilet paper every week. One toilet paper installed on the dispenser and two rolls placed under the sink. A physical plant inspection was made to resident rooms to confirm resident bathrooms are supplied with toilet paper. Based on the information obtained, there was insufficient evidence to corroborate the allegation of residents being charged for general use of hygiene supplies. Therefore, the allegation is deemed Unsubstantiated at this time.
