Licenisng Program Analyst (LPA) Michael Cava conducted a subsequent complaint visit to the facility to close out the investigation regarding the above allegations. During the course of the day's visit, a physical plant insepection, interviews with residents and staff, and record review were made.

Residents are not getting showers as needed:
In regards to the allegation, on or about 04/17/21, it was identified that there was a problem with the hot water heater. As a result, residents have been going without showers. LPA Tan initially conducted this investigation on 05/14/21 and he discovered that a plumber was called immediatley to address the problem and service was placed to repair two out of the three broken water pumps on 04/22/21. The third water pump, which was still functional, was replaced on 04/23/21. To address the resident scheduled showers for those whose rooms were affected by having no hot water, residents were taken to the vacant rooms, where there was hot water in the bathrooms, and provided their showers there. No complaints

or concerns regarding an interruption of showers were made. LPA interviewed ten of ten residents, but they could not recall the incident of having no hot water, or having to take a shower in an alternate room while repairs are being made. Based on the information obtained, there was insufficient evidence to corroborate the allegation of residents not getting showers as needed. Therefore, the allegation is deemed Unsubstantiated at this time.

Staff do not have sufficient training:
In regards to the allegation, it was alleged that the licensee hired inexperienced personnel and did not provide them training on handling residents that are diagnosed with dementia. During the visit, LPA reviewed five out of five staff records to insure training requirements for dementia care is met. Interviews with five of five staff was also made to confirm they've met this requirement. Based on the information obtained, there was insufficient evidence to corroborate the allegation of staff not having sufficient training. Therefore, the allegation is deemed Unsubstantiated at this time.

Administrator is not effectively managing facility:
In regards to the allegation, it was alleged that the previous administrator, refused to meet with residents who requested to meet with them. Interviews made with Petra Vancine, Resident Care Coordinator (RCC) reveal that the administrator has an open door policy with the residents, and speaks with them anytime during her scheduled working hours, which is Monday to Friday, 9am-6:30pm, and some weekends. There is a suggestion box located at a table on the second floor by the Activity Room where residents can write a note and drop off their concerns. Facility also holds monthly resident council meetings every last Tuesday of the month at 2pm for the residents to address any concerns with the facility. Petra stated that she hasn't heard of any negative feedback or concerns from resident council. LPA interviewed ten of ten residents who did not express any complaints with how the administrator is managing the facility. Based on the information obtained, there was insufficient evidence to corroborate the allegation of administrator not effectively managing the facility. Therefore, the allegation is deemed Unsubstantiated at this time.
