LPA's interview with the Activity staff on 08/04/21 at 2:45 PM confirmed that the activity was continuous in memory care despite the absences as the Executive Director and other Activity staff stepped in to assist the residents in memory care. LPA's observation during the initial visit on 08/04/21 at around 1:30 PM also revealed that some of the memory care residents were doing Videoke and others watching movie via internet based TV system in the memory care area.
Regarding the allegation that the facility is not maintaining safe environment for residents, it was alleged that a resident of memory care unit has a large dog which is unsafe for the other residents. LPA investigated a similar allegation (please see report dated 08/21/22) on complaint control no. 31-AS-20210728163608 and was unsubstantiated. The dog of the resident was documented as an emotional support dog and did not bother anyone at the facility as it stays on it's owner's room most of the time. LPA's interview with the Memory Care director today at around 12:00 PM, revealed that the dog is still in the facility and had not bothered any staff or resident of the facility nor she received any report about the dog misbehaving at anytime.
Based on the information gathered during this and prior visit, the allegations are deemed unsubstantiated at this time.
