Licensing Program Analyst (LPA) Sandra Urena conducted a subsequent unannounced visit to investigate the allegations listed above. The LPA met with the Administrator Emma Arutunina and explained the reason for the visit.

LPA Urena, along with the Administrator, toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that facility is in compliance with Title 22 Regulations. No health or safety hazards were observed at this time.

On 04/08/2026, Licensing Program Analyst (LPA) Sandra Urena conducted an initial unannounced visit to investigate the allegations listed above. The LPA met with the Administrator Emma Arutunina and explained the reason for the visit. LPA Urena interviewed residents at approximately 1:45 p.m. and requested records pertinent to the investigation.

Staff left resident in soiled diaper/clothing for an extended period of time.
To investigate the allegation that the residents were left wearing soiled clothing for an extended period of time, LPA Urena interviewed the residents (R1). R1 was found to be alert and oriented, and able to understand and answer the questions asked. The R1 reported that they do get changed often or as necessary and normally do not have to wait long to be changed if needed. R1 added that they understand that staff may be busy with other residents, when they request assistance to be changed out of soiled disposable underwear. Staff interviews revealed, “they change residents as needed and check on them every two hours.”
Based on the information received through interviews, there is not sufficient evidence to support the allegation that residents are left in soiled clothing for extended periods of time. Therefore, the allegation is deemed Unsubstantiated at this time.

Staff are not providing adequate supervision to the residents in care.
To investigate the allegation that staff are not helping reposition the resident, LPA Urena interviewed R1. The R1 reported that they do get assistance throughout the day, and at night R1 does not need anyone to reposition them. The R1 reported that staff are very attentive and that S1 is “amazing”.
Based on the information received through interviews, there is not sufficient evidence to support the allegation that staff are not providing adequate supervision. Therefore, the allegation is deemed Unsubstantiated at this time.

Staff did not respond to emergency alarm.
To investigate the allegation that the staff did not respond to the emergency alarm, the LPA attempted to conduct residents’ interviews. R1 reported that there was a night when the alarm started to make a beeping sound but not continuously. The night staff seemed not to notice the beeping sound. The sound then stopped. It hasn’t happened again. Other residents were not aware that the emergency alarm went off during the night or any other time.
Although the allegation may have happened or is valid, based on the interviews, there is not sufficient evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is Unsubstantiated at this time.

No citations were issued. Exit interview was conducted. A copy of the report was issued.
