On 02/09/2026 at 09:45 AM, Licensing Program Analyst (LPA) Lorena Casillas conducted an unannounced subsequent visit to issue the findings of the above listed allegations. LPA was greeted and granted access to the facility by staff. LPA met with Administrator, Anthony Powell, and explained the reason for the visit. Entrance interview conducted.

On 09/02/2025, the Woodland Hills South Adult and Senior Care Regional Office received a complaint regarding the allegations: Staff chemically restrained resident in care, Licensee is falsifying personnel records, Staff do not ensure that balanced meals are provided to clients, Licensee does not ensure that resident's needs are being met while in care. An additional allegation: Uncleared adults are working at the facility, was also reported, however this allegation was addressed on 09/10/2025 and will not be included in this report.

On 09/10/2025 an initial visit was conducted by LPA Casillas and North Los Angeles Regional Center (NLARC) Community Services Specialist (CSS) Rachel Cooper. On that day LPA conducted a tour of the facility, interviewed facility staff, interviewed Licensee, interviewed clients and obtained copies of pertinent information related to the investigation. At the time of the visit an interview was conducted with then Administrator Vashan Bobney.

Allegation #1: Staff chemically restrained residents in care.

This allegation investigation was conducted by Jose Santana, Investigator from Community Care Licensing Division of Investigations Branch (IB). The investigation consisted of interviews by IB with three (3) witnesses, eight (8) facility staff to include one on one staff (1:1) as some staff work both as DSP and 1:1, facility Administrator Bobney, three (3) out of four (4) facility clients, eight (8) hospital staff/specialists, reviews of Client #1 (C1)’s medical records, police records, and facility program plan.

It was alleged that around June and July 2025 facility staff were chemically restraining C1 with medication that was not prescribed to C1. During the investigation C1 denied being given medication that matched the medication in question’s appearance. IB determined through extensive medical record reviews and interviews that this investigation did not find sufficient evidence to prove C1 was given unprescribed medication. It was also stated by C1’s psychiatrist that the medication in question could not act as a chemical restraint because it only has mild melatonin-like effects. The medication in question additionally would not have had any drug interaction with C1’s prescribed medications. Medical records revealed that during the period in question, June-July 2025, C1 did not present with injury when examined at the hospital, with the exception of preexisting conditions. Therefore, based on interviews, record reviews, and observations the allegation is deemed unsubstantiated.

The remaining allegations listed below were investigated by LPA Casillas.

Allegation #2 Licensee is falsifying personnel records.

It is alleged that Licensee is falsifying personnel records. Regarding this allegation, it is reported that Licensee falsifies the number of hours that staff are present in the facility. LPA interviewed four (4) out of four (4) clients who denied this allegation stating that staff are at the facility at all times and they all confirmed that clients that need one on one (1:1) staff have them at all times as well as facility staff being present. LPA interviewed five (5) out of five (5) staff present and they all denied the allegation stating that they only get paid for the hours that they have worked. LPA interviewed Administrator Bobney who denied the allegation stating that hours are not falsified, adding that some facility staff also work for 1:1 company Right Choice who is vendorized through NLARC, this could potentially cause confusion as some staff work facility hours and also work for the 1:1 company. LPA interviewed Licensee Anthony Powell who also denied the allegation stating that they have a payroll company that takes care of providing payroll services. Additionally, Licensee states that it would make no sense to falsify the hours worked because that means that Licensee would have to pay staff for hours that they did not work. LPA reviewed staff records to compare the staff names to the vendor log and discovered that some facility staff also works for vendor Right Choice as 1:1 staff for clients. LPA reviewed client IPP’s and verified that clients that need 1:1 staff have said staff available. It was also observed that the number of staff present at the facility matches the number of staff required based on client needs. Therefore, based on interviews, record reviews and observations this allegation is deemed unsubstantiated.

Allegation #3 Staff do not ensure that balanced meals are provided to clients.

It is alleged that staff do not ensure that balanced meals are provided to clients. Regarding this allegation, it is reported that facility staff use facility funds that are meant to be used for purchasing food for the clients on food items for themselves and whatever is left over is for the clients. LPA interviewed four (4) out of four (4) clients who denied this allegation stating that they have no concerns with the food that is provided, adding that the amount of food provided is also sufficient. LPA interviewed five (5) out of five (5) staff present who denied the allegation stating that the clients all get the balanced meals that are notated on the menu as well as plenty of snacks and that if clients want additional food or something different, then it is provided.

All staff also stated that meals are prepared by different staff members that know how to cook, making the meals appetizing for the clients. LPA interviewed Administrator Bobney and Licensee who both denied the allegation stating that one of the most important expenses the facility has is the food. Both stated that they strive to provide the clients with balanced meals so that they can maintain good health however, sometimes the clients will choose to eat fast food and forgo the meals provided. Licensee added that the amount of money provided for groceries has matched the amount of food purchased and present at the facility. Licensee also stated that staff are encouraged to share meals with the clients, further motivating staff to provide balanced, tasty and nutritious meals. LPA reviewed the facility menu and observed that food items that are listed on the menu are present in the facility. During the facility tour, LPA observed there to be two (2) days of perishable and seven (7) days nonperishable foods along with plenty of snacks for staff and clients. Therefore, based on record reviews, interviews and observations this allegation is deemed unsubstantiated.

Allegation #4 Licensee does not ensure that resident's needs are being met while in care.

It is alleged that Licensee does not ensure that resident's needs are being met while in care. Regarding this allegation it is reported that staff do not take clients on outings and that clients are not provided with 1:1 care leaving them to care for themselves. LPA interviewed four (4) out of four (4) clients who denied this allegation stating that clients are often taken on walks and taken out in the community. They also stated that three (3) out of the four (4) clients have 1:1 staff all the time. Interview with five (5) staff members stated that three (3) out of the four (4) clients have 1:1 staff specifically for them and provide care as specified on their IPP’s. They also stated that clients are encouraged to go on outings and to join different activities, however, the clients refuse and choose to stay in their rooms. Additionally, staff stated that clients are encouraged to do things independently and only require prompting or reminders to do things such as bathing or doing laundry. If the client refuses, staff try other methods of prompting, or they take over certain tasks. Interview with Administrator Bobney revealed that in the past the clients have been encouraged to join a day program, but the clients have refused.

Administrator Bobney stated that the clients cannot be forced to participate in activities, but that staff are constantly encouraging them to go on walks or on outings. Administrator Bobney denied that clients are not provided with 1:1 care pointing out that the reason for the number of staff present during the initial visit is precisely to provide clients with 1:1 care as their IPP specifies. 1:1 staff members encourage clients to try to do things independently, however, 1:1’s will intervene when needed. LPA reviewed activities calendar and through interviews with clients LPA was able to confirm that activities listed did take place. LPA also reviewed all client IPP’s to ensure that the number of staff at the facility reflects the specifications for each client’s needs. Based on observations, interviews and record reviews, this allegation is deemed unsubstantiated.
