Kitchen Refrigerator(s) and freezer(s) are clean and large enough for the storage of at least two (2) days of perishable foods. Freezer is 0 degrees Fahrenheit. Refrigerator is a maximum of 45 degrees Fahrenheit. LPA observed supplies of nonperishable foods for a minimum of one week and fresh perishable foods for a minimum of two days. Located in the rear pantry behind the kitchen. There are sufficient amounts of tableware, tables, dishes, and utensils. There are sufficient amounts of equipment for the storage, preparation, and service of food. All kitchen, food storage, and preparation areas are clean. LPA observed knives are pad locked on an over-the-counter cabinet. Cleaning supplies locked under the sink, additional toxins are stored secured in supply closet.
LPA observed that Medications were safe, locked and inaccessible. All medications observed were labeled and maintained in compliance with label instructions and State and Federal law. All Medication are stored in a black Husky industrial cabinet near the kitchen.
Common areas were clean and appropriately furnished. These included the living room and dining area.
Documents are posted as mandated. Last Disaster drill and emergency drill are conducted monthly. The first aid kit is fully stocked with manual. Smoke and carbon monoxide detectors are hardwired and in compliance and operational. Two (2) Fire Extinguishers were observed throughout the facility purchase date is April 4th, 2023. Fire extinguishers are in the hallway and kitchen.
At 10:20 AM LPA Alemoh began a facility file review.
Seven (7) staff records were reviewed, 7 out of 7 staff records had current first aid certificates and had required criminal record clearances or criminal record exemptions. Four (4) client records were reviewed and, 4 out of 4 client records had Admission Agreements, Medical Assessments, Pre-appraisals (or Reappraisals) and/or Needs & Services Plans. LPA Alemoh reviewed P&I money, 4 out of 4 residents P&I were intact and were not commingled with facility funds or petty cash. All client/resident and staff files are locked in small rolling cabinet in the dining room inaccessible to residents.
No deficiencies cited.
An exit interview was conducted. A copy of this report and appeal rights were discussed and left with Administrator Marian Ukwamedua.
