25 | At approximately 2:10 p.m. on 04/10/2025, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced annual inspection. LPA met with the administrator and disclosed the reason for the visit.
A file review was conducted prior to today’s visit.
The facility was last visited on 03/27/2025 for a case management visit. It is a single story building with four (04) bedrooms, two (02) bathrooms, kitchen, laundry room, common areas, and outdoor areas. It has an approved fire clearance for four (04) ambulatory clients.
LPA observed a maintained front yard. A screening station at the front contained surgical masks, sanitizer, a digital thermometer, and a visitor log. Postings at the front included COVID precautions, facility license, confidential complaints contacts, emergency disaster plan, personal rights, and the facility sketch with evacuation routes labelled. An activity calendar and weekly menu were also posted.
Walls, floors, windows, screens, and blinds were clean and in good repair. The living room contained furniture in good repair, reading material, games, darts, and a television. At 2:30 p.m. LPA measured the room temperature to be 73 degrees Fahrenheit. A hallway closet contained hygiene supplies and fresh linens.
LPA observed an adequate supply of perishable and non-perishable foods in the kitchen and the garage. Appliances were in good condition. cleaning solutions were locked under the sink. Sharps were locked under the counter in a lockbox. The stove surface and hood were clean. A washing machine and dryer were located
