On 10/24/2025 Licensing Program Analyst (LPA) Melissa Spaeth conducted a subsequent complaint investigation at the above facility to deliver findings for the above allegation(s). LPA Spaeth met with the staff member, Paulette James. LPA explained the purpose of the visit was to deliver the findings.

The investigation consisted of the following: On 7/31/2025 LPA Spaeth conducted an unannounced visit, and toured the facility from 2:15 pm until 2:30 pm. LPA Spaeth interviewed the Administrator and three out of nine staff members (S1-S3) on 10/14/2025 at 10:00 am until 11:00 am. LPA also interviewed two out of four clients (C1-C2) on 10/20/2025 at 2:00 pm until 3:00 pm.

Regarding the allegation: Staff do not follow proper sanitation protocol. It is alleged a staff member used a sponge and spray bottle to clean the sink, counter tops, and the dishes, which poses a health and safety hazard to residents in care. During interviews with staff, S1-S3 denied the allegation. During interviews with residents, one out of four clients confirmed this occurred and one client confirmed this did not occur.

Regarding the allegation: Staff did not ensure resident attended their dental appointment. It is alleged staff did not provide a client transportation to an appointment due to a staff error. During interviews with the administrator and staff, S1-S3 and the administrator denied the allegation. During interviews with residents, one out of four clients confirmed this occurred and one client confirmed this did not occur. C1 did admit their appointment was rescheduled and the staff took them to the appointment a few days later.

Based on interviews, there is insufficient evidence to verify the allegations. The allegations are hereby unsubstantiated.
