At 10:00am, Licensing Program Analyst (LPA) Angela Panushkina conducted an unannounced visit in response to the above-mentioned allegations. LPA met with the Administrator and explained the reason for the visit.

At 10:05am, LPA requested resident and staff roster. At 10:10am, requested copies of pertinent information which include, but not limited to Admission Agreement, Physician's Report, Appraisal Needs and Services Plan, Menu, etc. relevant to the investigation. At approximately 10:20am, LPA conducted a physical plant tour, to ensure health and safety of the residents are protected. Between 10:30am - 11:30am, LPA conducted an interview with the Administrator, one (1) staff and two (2) out of three (3) residents who were able to communicate.

Allegation: Staff does not ensure meals are nutritionally balanced for residents in care

It was alleged that staff do not ensure meals are nutritionally balanced for residents in care. To investigate this allegation, LPA conducted an interview with the Administrator and was informed that all meals are prepared according to the facility’s posted menu and are designed to meet residents’ nutritional needs. The Administrator reported that staff follow the planned menu and ensure meals contain appropriate portions of protein, vegetables, fruits, and grains. Interview with one (1) staff member revealed that meals are prepared based on the facility menu and dietary guidelines, if any. Staff reported that residents are offered balanced meals daily and substitutions are made only when medically necessary or based on resident preference. Staff denied serving meals that were not nutritionally balanced. Two (2) out of three (3) residents interviewed expressed no concerns regarding this allegation. During the visit, LPA observed the lunch meal prepared for residents. The meal included a protein item, a cooked vegetable, fruit, and a grain/starch component. The meal appeared well-portioned and consistent with a nutritionally balanced menu. Based on interviews and direct observation, there was insufficient evidence to support the allegation that staff do not ensure meals are nutritionally balanced for residents. Therefore, the allegation is Unsubstantiated at this time.

Allegation: Staff served undercook meat to residents in care

The investigate this allegation, LPA conducted an interview with the Administrator who denied the allegation and stated that all meat served at the facility is fully cooked and prepared according to safe food-handling practices. The Administrator stated that staff are trained to check food temperatures and ensure meat is fully cooked. One (1) staff interviewed confirmed this and added that any residents’ concerns about meals would be addressed immediately with a replacement or alternative. Two (2) of the three (3) residents interviewed denied ever receiving undercooked meat and reported that meals are generally well prepared. No residents expressed concerns regarding this allegation. During the visit, LPA observed the lunch meal prepared for residents, which included a fully cooked protein entree. The meat appeared properly cooked, with appropriate texture and temperature. No undercooked food items were observed. Based on interviews and direct observation, there was insufficient evidence to support the allegation that staff served undercooked meat to residents. Therefore, the allegation is Unsubstantiated at this time.

No deficiency issued during today's visit.
