32 | Interviews with three (03) out of three (03) other clients in the home revealed they had never been threatened with eviction and they never heard S1 threaten C1 with eviction. Telephonic interview with C1’s service coordinator, Witness #1 (W1), at 12:45 p.m. today revealed S1 and the administrator discussed C1’s eviction, although it never happened and no eviction was issued. Record review of C1’s files revealed no pertinent information to the investigation. Based on interviews and record review, there is insufficient evidence to confirm the allegation. Therefore, the allegation is deemed UNSUBSTANTIATED at this time.
Regarding the allegation “Staff do not provide adequate food service to client in care" it was alleged S1 cooks for other clients but does not cook for C1. Interview with C1 revealed S1 cooks for C1 when they request it. Interview with S1 revealed two (02) out of four (04) clients cook for themselves, so S1 usually cooks for C1 and Client #2 (C2). S1 cooks for all clients upon request and always ensures they are fed and the facility maintains adequate supplies of food. Interviews with three (03) out of three (03) other clients in the home revealed they either cook for themselves or eat what S1 cooks. Zero (00) out of four (04) clients stated S1 denies clients food. During today’s facility tour, LPA observed adequate supplies of perishable and non-perishable food. Record review of C1’s care plan revealed C1 “can… prepare something to eat if hungry” and “can prepare [their] own meals without any assistance, but [they prefer] when someone cooks”. Based on observations, interviews, and record review, C1 is capable of preparing meals for themselves. S1 cooks for C1 and maintains adequate food supplies in the home. Therefore, the allegation is deemed UNSUBSTANTIATED at this time.
Regarding the allegation "Staff does not treat client with dignity and respect " it was alleged S1 makes fun of C1’s disability and accuses them of breaking things in the home. Telephonic interview with C1 revealed S1 tells C1 that they do not have a disability. C1 confirmed they get incorrectly accused of breaking household items. Interview with S1 revealed they do not tease or disrespect C1. S1 learned from Client #3 (C3) that C1 had broken a broom while angry, so S1 addressed the issue with C1. Interviews with three (03) out of three (03) other clients in the home revealed they are treated with dignity and respect by S1 and other staff. Zero (00) out of three (03) clients have heard S1 disrespect, accuse, or tease C1. LPA observed S1 treating clients with respect during today’s visit. Record review revealed no pertinent information to the investigation. Based on observations, interviews, and record review, there is insufficient evidence to verify the allegation. Therefore, the allegation is deemed UNSUBSTANTIATED at this time.
