32 | Regarding the allegation: Staff do not treat residents with dignity and respect. It was alleged that Resident 1 (R1) was verbally and physically injured by staff. To investigate the allegation, LPA attempted interviews with one (1) resident and three (3) staff members. LPA’s interview with S1 revealed R1 had two (2) incidents where they were found away from the facility. Per S1, on (7/17/2026), R1 attempted to leave the geographic area where staff intervened to bring them back to the facility and upon their return, R1 displayed aggressive behavioral episodes with self-harming statements. On (7/19/2026), S1 revealed R1 had once again left the facility where again staff tried to intervene due to heat exposure, police were called, and R1 was taken to the hospital where they were later placed on a “5150 hold”. LPA’s interview with both S2 and S3 correlated with S1’s interview. LPA’s interview with S2 revealed R1 was observed to have a nosebleed on said incident which could have been related to heat exposure. Per S2, when staff tried to assist R1 back to the facility, R1 refused and continued to make self-harming statements such as, “I want to kill myself”. When LPA questioned if R1 was verbally and physically abused by staff, all three (3) staff denied the allegation. LPA attempted to interview R1, but they have not returned from the hospital since said incident.
LPA’s record review of information provided by the Reporting Party’s (RP) confirmed R1 was placed on a “5150-hold” due to them making, “…incoherent statements…” and insisting they would, “kill themselves”. LPA’s record review of R1’s Physician report revealed R1 to have various diagnosis including psychological diagnoses. Further record review of R1’s Physician Report documented them to be, “Depressed” under their Mental/Cognitive Condition Assessment. Additional record review of the Facility’s Unusual Incident/Injury Reports (SIRs) confirmed the facility had self-reported the incident to Community Care Licensing Division (CCLD) on 7/26/2026. LPA’s supplementary record review of the weather forecast of said incident (7/19/2026) confirmed weather forecast to document highs of 94°F. During LPA’s physical plant tour, LPA did not observe staff mistreating residents verbally nor physically.
Based on interviews, record review, and observations there is not enough information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time.
