On 2/20/2026 Licensing Program Analyst (LPA) Evelin Rios conducted an unannounced complaint visit to this facility to investigate the above mentioned allegations. LPA met with staff that granted access. LPA met with the Administrator Designee, Daniel Morales shortly after and LPA explained the purpose of this visit.

The investigation consisted of the following: A physical plant tour was conducted to ensure the health and safety of clients in care. LPA did not observe any health or safety concerns or issues. From 8:50 a.m. to 9:23 a.m., LPA interviewed two (2) of the four (4) clients. One (1) client refused to be interviewed, and the other was at day program. From approximately 9:25 a.m. to 1:00 p.m., LPA interviewed eight (8) staff members, including the administrator designee and the licensee, Audrey Hughes.

LPA requested and obtained copies of the following documents: Register of Facility Clients (LIC 9020), Personnel Report (LIC 500), clients’ Individual Program Plans (IPP), Daily Logs, Special Incident Reports (SIR) for the date in question, along with the schedule and a client’s Individualized Support Plan (ISP). At 3:44 p.m., LPA interviewed a client's family member that witnessed clients behavior outside of the facility.

Allegation: Staff pushed resident. It was alleged that staff pushed a resident to the ground on Sunday, 2/08/2026. LPA's interview with two (2) clients revealed that they had not been pushed by staff, nor had they witnessed staff push another client. Interviews with staff who were present on Sunday, February 8, 2026, revealed that during the afternoon shift three (3) clients wanted to go out. Staff asked clients to wait until the next shift so that everyone could go to the same location as there was only one car available. This led one (1) client to become upset, which then triggered similar reactions from the other (2) clients in the facility. According to staff, two (2) clients ended up outside in the neighborhood. One (1) client was being verbally aggressive, hitting cars, and the other client was hitting a staff. During the incident, two clients experienced falls outside the facility. Both clients corroborated that their falls were not the result of being pushed. Staff denied pushing any clients or witnessing other staff push clients. LPA’s review of the Daily Logs and Special Incident Reports (SIRs) corroborated staff statements. Additionally, LPA’s review of the Individual Program Plan (IPP) for the (2) clients who were outside revealed that the clients may have episodes of yelling and verbally aggressive outbursts, which is consistent with the behaviors described by staff. However, only one client's IPP indicated they would become physically aggressive. Staff reported that they had not witnessed the clients be physically aggressive while at this facility.

Based on interviews and record review there is insufficient evidence to support the allegation, staff pushed resident. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.

Allegation: Staff speak inappropriately to residents. It was alleged that there is verbal abuse. LPA's interview with two (2) clients revealed one (1) client has concerns regarding one (1) staff making fun of them and laughing at them. The client indicated it was only towards them and had not witnessed it happen to other clients. The other client did not indicate they had witnessed staff speak inappropriately to clients or have been spoken to in that manner. LPA's interviews with four (4) out of eight (8) staff corroborated the allegation, stating they had witnessed staff being rude to a client. However, their statements were inconsistent regarding which staff member engaged in the behavior and what that behavior was. Eight (8) staff denied speaking inappropriately to clients.

Based on client and staff interviews, there is insufficient evidence to support the allegation, Staff speak inappropriately to residents. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.

Allegation: Staff did not follow reporting requirements. It was alleged that the administrator received a report that a client had been antagonized and pushed to the ground and failed to report the information to Community Care Licensing (CCL). LPA’s interview with the administrator revealed that they had been informed by a staff member. According to the administrator and LPA's review of the staffing schedule for that day, the staff member who reported the information was not present during the incident on Sunday, 02/08/2026. This staff member stated that a client had reported being pushed by staff. The administrator reported that they interviewed the client on Tuesday, 02/10/2026, and the client denied they were pushed, reiterating earlier statements that they were upset because they wanted to go out into the community and were told they couldn't. According to the administrator because the client denied the incident occurred it was not reportable. LPA's review of the SIRs emailed to CCL and North Los Angeles County Regional Center indicated they were submitted to the agencies timely and consisted statements the client made during the incident on the telephone with 911.

Based on interviews and record review there is insufficient evidence to support the allegation, Staff did not follow reporting requirements. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.
