At approximately 9:00 a.m. on 03/03/26, Licensing Program Manager (LPM) Naira Margaryan and Licensing Program Analysts (LPAs) Michael Cava, Antonia Alvizar-Ettima, and Nicholas Reed conducted an unannounced case management visit in conjunction with a subsequent complaint visit. LPAs and LPM met with Staff #1 (S1) and disclosed the reason for the visit.

Today’s case management visit was conducted to address previously noted concerns noted in the facility. On 10/20/25 the Department issued an order of immediate exclusion for the facility administrator Lala Soghomonyan. LPAs Michael Cava and Jose Gary Tan hand-delivered the immediate exclusion letter to staff Matthew Caceres. Matthew informed LPAs that the Licensee was in the process of hiring a new certified administrator to oversee facility operations. During that visit, LPAs noted various Title 22 deficiencies and issued citations and civil penalties.

On 02/18/26 LPAs Michael Cava and Leslie Ngo-Castaneda conducted another Case Management visit to the facility to ensure a new Administrator was hired. Between 8:30 a.m. and 10:00 a.m. on 02/18/26 LPAs interviewed S1 and four (04) residents. Nobody knew who the licensee or administrator were. Between 9:30 a.m. and 10:30 a.m. on 02/18/26 LPA Cava made multiple attempts to contact the licensee on file but received no response.

On 02/26/26 LPA Cava made a Case Management visit and met with the same facility staff, S1. All residents were observed still residing in the facility. Additional civil penalties were issued. S1 was again reminded that all residents must be relocated. No deficiencies were corrected as of 02/26/26 and today. Additional civil penalties were issued for repeat violations and fire clearance violations. LPA Cava issued a “7-day notice of closure” to the Licensee via regular and certified mail and email, informing that the Licensee must contact the Department within seven (07) days. Failure to respond would consider the license forfeited.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Licensee abandonment of the facility posed an immediate risk to the Health and Safety of the residents in care. Since 10/28/25 the facility operated without a qualified administrator. The Department received no response from the 7-day closure notice.

Based on the information above, it is concluded that the facility is abandoned and the license shall be forfeited by operation of the law. A Notice of Violation of the Law (NOVL) Letter was issued to S1 along with a copy of today’s report. The NOVL letter was also emailed to the facility email address on file.

Additional deficiencies issued from today’s visit include deficiencies for administrator qualification and duties and acceptance and retention limits.

LPAs and LPM were unable to contact the licensee after multiple attempts between 9:00 a.m. and 1:00 p.m. today. LPM Margaryan spoke to the father of the licensee at 10:30 a.m. to discuss the staffing situation at the facility. LPM was informed no other staff could come to the facility and the licensee did not want further involvement with the facility. A citation was issued under Health and Safety Code 1569.19(1) for facility abandonment by the Licensee. The staff was advised that all residents must be relocated from the facility within two weeks.

LPAs interviewed three (03) residents and S1 between 9:00 a.m. and 11:30 a.m. Around 11:15 a.m., Resident #1 (R1) indicated that they had not received medication for their diabetes since admission three weeks ago. R1 also had been left in feces since yesterday, 03/20/26. Interview with S1 at 11:30 a.m. confirmed they had not yet changed R1 because they had a large sore on their back that was infected and contagious. R1 called 9-1-1 at approximately 12:45 p.m. Paramedics assessed R1 and two (02) other residents. Since S1 packed up their belongings and stated they were ready to leave the facility, residents were considered abandoned and neglected. All three (03) residents were transported to the hospital.

Exit interview conducted. Appeal rights discussed. Copy of report provided.
