Licensing Program Analysts (LPA’s) Kelly Dulek and Erica Mosley arrived at the facility unannounced to conduct a required annual visit at 8:53 a.m. The LPA’s were greeted by staff and informed them of the reason for the visit. Administrator Jennifer Enriquez, Chief Clinical Officer/LMFT Sharon Volner and Director of Operations/LVN Wendy McCain arrived shortly thereafter.

The LPAs along with the Administrator, Chief Clinical Officer and Director of Operations toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations.

KITCHEN: Kitchen appliances were in operable condition. The facility has a sufficient supply of seven (7) days perishable and two (2) days non-perishable food. Additional food supplies and emergency food supply is kept in the attached garage. The LPAs observed the entrance closet where all medications and sharps are kept locked and inaccessible. Client files are locked and inaccessible in the staff office. BEDROOMS: Bedrooms were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. There are three (3) designated client rooms that are shared. There was a linen closet in the hallway with extra towels and linens. BATHROOMS: There are three (3) bathrooms for client use and one (1) for staff use that remains locked at all times. Client bathrooms are clean and sanitary and in operating condition. Bathrooms are sufficiently stocked with hand liquid soap and paper towels. Bathrooms were measured for hot water, the first bathroom measured at 117 degrees Fahrenheit at 9:30 a.m., the second bathroom measured at 106.4 degrees Fahrenheit at 9:37 a.m.

TREATMENT/THERAPY, MEDICATION ROOM & OFFICE AREAS: There is a living room, Zen Den /meditation room, Rec room with a gym and an office/ administration of meds room. The office/ administration of meds is kept inaccessible to clients unless properly supervised. Refrigerated medications are stored in a fridge in the office/ medication administration room. The first aid kit was observed to be complete.

INFECTION CONTROL/DISASTER PREPAREDNESS: During today’s visit, the LPA's reviewed the facility's infection control policy and disaster preparedness policies. Emergency/ disaster drills are conducted quarterly The last drill was on 07/09/2024 All items reviewed were in compliance.


GROUNDS: There is a two (2) covered patio area with patio furniture including a table and chairs for client use, benches and a water fountain/fall. Facility has one (1) fenced gate that self-latches with clear passageways for emergency exit use. COMMON SPACES: The living and dining areas are clean and properly furnished with seating, a table, and television for client use. Fire extinguisher was observed to be last serviced on 10/27/2023. The facility maintained a comfortable temperature. Smoke detector(s) and carbon monoxide detector were tested at 1:45 p.m. and operational at the time of the visit. The LPA’s observed required postings throughout the common spaces. There is a laundry room that contains a chemical cabinet and cleaning supplies that is locked and inaccessible to clients. The garage is attached and is used as a gym room and rec room. RECORDS: Client records review began at 10:08 a.m., five (5) client records were reviewed for, but not limited to care plans, medical records, admissions agreement, consent forms. All records were in order. Five (5) personnel records were reviewed for, but not limited to health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All records were in order. MEDICATIONS: Medications review began at 10:40 a.m.; medications are centrally stored and locked in a closet at the entrance of the facility. Medications are labeled and checked for expiration dates. Medications were reviewed for two clients. Medications were properly labeled and appeared to be administered as prescribed.

INTERVIEWS: During today's visit, LPA interviewed one (1) client and four (4) staff. No concerns noted.

No deficiencies cited. Exit interview conducted and a copy of the report issued.
