32 | Licensee Diegues admitted placing the tumbler on the play equipment was a mistake, which caused C1 to sustain burns on chest to lower naval. The investigation revealed the licensee violated Tittle 22 Regulation, Personal Rights by exposing C1 to unsafe conditions that resulted in serious injuries.
Based on the information gathered during the investigation, the complaint allegation, Child sustained first and second degree burns due to inadequate supervision, is deemed Substantiated. A finding of substantiated means the preponderance of the evidence standard has been met. An immediate civil penalty of $500.00 is hereby assessed.

Facility shall post the Notice of site visit for 30 consecutive days as there is immediate risk to the health and safety of personal rights of children in care. Also, facility was informed to provide a copy of this licensing report dated 12/19/2025 to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care and to any newly enrolled parents/guardians for the next 12 months from the date of this report.
A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal rights were provided.
A copy of this report must be made available to the public upon request for the next 3 years
