On May 21, 2026, Licensing Program Analysts (LPAs) Ali and Charon met with Licensee Claudia Landaverde, who guided analyst on a tour of the facility for the random annual inspection. This is a one story, 4 bedroom, 2 bathroom home with kitchen/dining, family room, living room, laundry room and garage. There is no pool/spa or body of water on the premises. Upon arrival LPAs observed 10 day care children in care. Licensee was present and Assistant #1 went to drop off two kids to school. Licensee is off ratio. Assistant #1 left at 8:36am and returned at 9:00am. Facility operations are Monday-Friday 6:30AM-6:30PM. LIC 9040 is current, 23 children enrolled. Incidental Medical Services (IMS) policy was discussed. Licensee states there are two IMS children. Licensee will develop an IMS plan to assist in the children care and email it to LPA Ali.

Physical Plant: The home was inspected inside and out for safety, clean and orderly, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds (laundry room has safety door knob), medicines (upper kitchen cabinet with safety latch) and hazardous items (sharp knives in kitchen drawer) that can pose a danger to children. Main care is provided in the living room and family room.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Children use the bathroom in hallway on the right. Children have access to the living room, family room, and dining room area. Off limit areas include all bedrooms (safety door knobs), bathroom #2, laundry room (safety door knob), and garage (past laundry room). Kitchen observed to be fully screened by two safety gates, allowing inaccessibility to kitchen area. LPA observed a fireplace in the living room to be fully screened. Safe and age-appropriate toys, play equipment and materials were observed. The smoke detector and carbon monoxide detector, Fire Extinguisher (2A10BC) is in the green and in operable condition (4/7/26 serviced date). Per Licensee, the fire marshal came last in 2023. Electrical outlets are inaccessible. There is a designated area for ill children as necessary near front entrance. Per Licensee there are no weapon/firearms and no smoking in the home. The facility sketch is complete and current; there is working telephone (cell). LPAs observed a bird in cage in the daycare room. There are no window cords accessible to children.

Fire/Disaster Drill is complete and maintained current. Last Fire/Disaster Drill was completed on 4/10/26 at 4:10pm (fire) and 5/18/26 at 3:13pm (earthquake).

Bathroom: Shower/tub are free of hazards (childcare bathroom). LPAs did not observe any hazardous items in the children's bathroom. Toilet and faucet are clean and operable.

Kitchen: The kitchen is off limits by two safety gates. LPAs observed the knife drawer needs safety latch replaced. Medication is stored in cabinet near sink inaccessible to children. Chemicals are stored in top cabinet near kitchen exit inaccessible to children. If food is brought from the children’s home, the container shall be labeled with the child’s name and properly stored or refrigerated. The home has a clean and fully stocked refrigerator/freezer.

LPAs observed medication on the refrigerator door. LPAs recommended for medication to be placed toward the back of refrigerator in locked container. Licensee currently enrolled in Ventura food program, last inspected in April 2026. Breakfast, lunch, AM and PM snacks and dinner are provided. Naps are provided on cots in the living room.

Outdoor: The backyard is safe for children. The backyard is completely fenced (with block cement). There is no body of water. There are two outdoor air conditioner fully surrounded by wooden fencing, inaccessible to day care children in care. LPA observed age appropriate toys and play equipment. Per licensee, there are three dogs on the premises and vaccinations are current. LPAs observed shaded barricaded area on the right side of the home for outdoor activities for the preschool age children. The back of the yard has two wood play equipment areas that are shaded. LPAs observed a shed that was not locked. Assistant #1 stated that he will place a lock on the shed to make it inaccessible to children. LPAs recommend the large wood play equipment to have padding surrounding it for children safety due to the concrete and hard ground below.

Advisory/Other: First Aid kit was observed with supplies readily available in bathroom. LPAs reviewed staff records (3), all incompliance. LPAs reviewed children records (3), all incompliance.

Documents Provided and or Discussed: Fire Drill Log, Roster, Postings, Safe Sleep PIN 20-24-CCP, Individual Sleeping Plan (LIC9227), and Safe Sleep Log. Licensee stated currently has child care insurance. LPAs observed a current drivers license and car insurance for Licensee and Assistant #1.

Criminal Record Clearance - Family Child Care Homes
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Safe Sleep - Family Child Care Homes
LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Deficiencies Cited: (See LIC 809D). The following One Type A deficiencies are being cited in accordance with Title 22 of the California Code of Regulations and/or Health & Safety codes.



A notice of site visit was given to licensee and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
