 On 07/27/2023 at 4:10PM LPA Alemoh arrived at the facility to amended the report on 07/19/2023. LPA observed 8 children during this visit. This complaint investigation report is amended to change the report confidential to public status .

On 07/19/2023 at 10:50 AM, Licensing Program Analyst (LPA) Andrew Alemoh met with licensee Theresa De Jesus Vasquez for the purpose to conduct interviews and deliver the finding of the above allegation. Upon arrival, LPA 4 preschool age, infants (4) and 2 staff (licensee and licensee husband).
The investigation consisted of interviews with the licensee, parents, children and other complaint relevant parties. The investigation revealed the following: there is no evidence of the children exposing himself to any day care children. Children interview statements corroborated with the other day care children, and staff statements. Children are provided a safe and respectful place to play. Licensee and licensee husband are providing supervision to all day care children during the hours of operation inside and outside the day care areas.

Based on the evidence obtained, the allegation of lack of supervision is unsubstantiated.
An exit interview was conducted, a copy of this report was provided along with the appeal rights
