On 7/19/2021, Licensing Program Analyst (LPA) Carol Heath met with the licensee, Dalisha Guidry and Maritza Heflin who granted access into the home. The purpose of the inspection is to conduct an unannounced Required 1 Year inspection at the above facility. Licensee is licensed to provide care and supervision for a Large family child care for the capacity of 14 children. There are currently 4 children enrolled in the family child care.

Currently residing in the home is the licensees (Dalisha and Maritza) and 2 minor children. The licensee states no other adults are residing in the home. LPA toured the home inside and out. Hours of operation are Monday through Sunday, 5:00 AM to 1 AM. Incidental Medical Services (IMS) policy was discussed.

The home is set up as follows:



This is a single-story house with 3 bedrooms, 2 bathrooms, with kitchen, dining room, family room, living room, and attached garage. Per Licensee Main care is conducted in the designated (playroom), which is located in the family room, near the main entrance door on the left side of the home. Age-appropriate toys and furniture were observed to be on the premises of this facility. All toys and furniture were observed to be in good condition during this inspection. There are educational floor mats observed on the playroom floor. A security gate was observed at the entrance of the playroom and another security gate near that separates the kitchen from the playroom. There is a storage closet near the entryway which children can store their items or belongings.

Kitchen: Sharp items such as knives and scissors are stored so they are inaccessible to children. Cleaning items are stored and locked. Children utilize a bathroom in the hallway, the bathroom was observed to be free and clear of hazardous items. The bathroom was observed to have a working toilet, sink, and an ample supply of towels and soap accessible for the children to use. The cabinets were observed to be locked by a child-proof latch which was observed on the front door of the cabinet.

Backyard: Children will have access to the backyard area. There is an artificial grassy area in the yard for active play. Several mature trees were observed in the backyard that provides shade for the children. Small play toys were observed in the backyard that was observed to be in good condition. There is a wooden shed located in the backyard which was observed located during the time of the inspection. The shed was observed to have additional play toys for children. AC/Heating unit observed on the backyard was observed without a covered. LPA also observed a trampoline in the backyard. LPA discussed the age and weight requirement with the Licensee.

The Licensee states the off-limit areas of the home are all bedrooms , laundry room and garage. The garage is used for storage only and no child care activities are conducted in the garage.

There is a fireplace located in the family room which is properly screened. The home has central AC and heat. Per licensee, there are no weapons or firearms on the premises. There is no pool or bodies of water observed on the premises. LPA observed there is a required fire extinguisher (2A10BC) fully charged and located in the kitchen inaccessible to children. The smoke detectors and carbon monoxide devices tested operable. The First Aid Kit was observed complete with supplies and a first aid manual.



This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies and reviewed children’s, personnel, and administrative records. · LPA observe both licensees has current Pediatric CPR and First Aid Training with expiration date (7/2023) 1 hour of nutrition training, (8) hours of Preventive Health and Safety Training.
· Both licensees have the required immunization.
· Both licensees have completed the online mandated reporter training at www.mandatedreporterca.com, and will renew 9/5/2021
· Licensee does not provide transportation for children.
· LPA reviewed 3 children's records, the records are incomplete and missing forms OR LPA observed 3 children files contained all required licensing documents.
· Per the licensee, fire and disaster drills are conducted every 6 months; the last drill was documented and conducted on 06/11/2021.

· LPA observed Child Care Facility Roster. Per Licensing Information System, facility annual fees is not current. LPA and the Licensee discussed the situation. LPA provided a phone number to call.


· Licensee has posted as required the Facility License, Emergency Disaster plan, and Parents Rights Poster. The facility roster is not current. there are no current facility earthquake/fire drills documents observed during the time of this inspection.

The following information was discussed with the licensee:


ü Mandatory Forms for the children’s files and provider’s files.
ü Requirements for fire drills, earthquake drills, and documentation for both.
ü The role and responsibilities of being a mandated reporter was discussed.
ü The licensee is reminded that 100% supervision is required to children at all
times.

ü Capacity requirements, Roster requirements, Posting requirements, Documentation requirements for disaster drills (fire and earthquake). Mandatory Forms for the children’s files and provider’s files, and Safe Sleep Awareness. The role and responsibilities of being a mandated reporter were reviewed. The licensee was reminded that supervision is always required for children in care.

ü Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified.


ü Licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B

ü The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot-line at 1-800-540-4000. Also call the CCL office and follow up with a written Unusual Incident/Injury Report (LIC 624B).

ü The licensee was informed that the presence of adults in the home without Criminal Record Clearance or Exemption will be cited and civil penalty assessed for $100 per day. The licensee may find additional information and forms on the DSS website at www.ccld.ca.gov including information on the Live Scan application (LIC 9163). Appointments can be made for Live Scan at 1-800-315-4507

ü The regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers, and any other items that fall into that category.

ü --Licensee was advised to visit the CCL website (www.ccld.ca.gov) to obtain updates of courses and updates/changes to the regulations.
