32 | Knives are kept in the off-limits kitchen in a drawer, inaccessible to children in care. Medication is kept in the off-limits upstairs bathroom, inaccessible to children in care. Cleaning supplies and chemicals are kept in the inaccessible garage and closet off-limit areas away from children in care.
There are age-appropriate toys and equipment on the premises. The outdoor area is free from sharp objects, broken toys/furniture & equipment, and other debris. The climbing structure, slides, and other outdoor play equipment are safely fastened to the ground with adequate cushioning material beneath them. Per the Licensee, there are no weapons, firearms, and ammunition in the facility. The LPA did not observe any weapons. Per the Licensee, there is no smoking on the premises.
The First Aid kit included a temperature thermometer, gauzes, and cleansing pads/solution was observed to be incomplete, the Licensee added the missing scissors making the First Aid Kit complete and returned the kit to the inaccessible off-limit cabinet. The required fire extinguisher (2A10BC) is reading in green. Smoke and carbon monoxide detectors were found to be in operable condition, tested at 10:10 a.m. Fire and Disaster drills are conducted at least every six-month, last drills were recorded on 4/19/2023 at 11:30am.
Licensee had all the required posted documents: Facility License (LIC 203, Notice of Parent's Rights Poster (PUB 394), Emergency Disaster Plan (LIC 610A), facility sketch, and Earthquake Preparedness Checklist (LIC 9148).
Licensee does not have insurance coverage for the facility, the Licensee has the parents complete the LIC 282.
The Licensee provided proof of immunization against pertussis (TDAP), measles (MMR), TB, and statement declining the Influenza for self, spouse, and staff.
Licensee was able to provide a valid CPR/First Aid training valid until 8/21/2023; however, the CPR/First Aid has never been completed for Staff 1. This is a Type B Violation, see the LIC 809D for the details. Child Care Provider Mandated Reporter Training Certificate has expired for the Licensee and Staff 1 and is no longer valid, this is a Type B violation, see the LIC 809D for the details. Staff Personnel files were complete with the: LIC 9052-Notice of Employee Rights, LIC 9108-Statement Acknowledging Requirement to Report Child Abuse, Immunizations: MMR, Tdap, TB, & Flu shots/statements declining the Flu shot, background clearances. |
