On Thursday, June 4th, 2026, Licensing Program Analyst (LPA) Evelyn Garcia conducted an unannounced Capacity Increase inspection. Upon arrival, LPA met with licensee Jenik Hairepetian. The requested capacity is 14. The fire department has granted a fire clearance dated 4/30/26. LPA Garcia observed the pull down fire alarm, near the stair case in the home. Licensee is advised that as a family childcare home operating at full capacity, they must adhere to the following: Operate with a Maximum of 14 children in care with no more than 3 Infants, also must have 2 school age children enrolled. Or Max. 12 Capacity with no more than 4 infants. A Qualified Assistant must be present when more than 8 children are in care. LPA went over the childcare ratios for a large family home with licensee and provided licensee a copy. There are no bodies of water on this premises.

Upon arrival, LPA Garcia did not observe children in care and licensee stated she has 8 children enrolled.Licensee presented evidence of property control. Operating hours are Monday- Friday, 7:30am- 5:00pm, however the licensee currently only provides care for children when the schools are out of session, such as breaks or holidays. All adults living in the home have been fingerprinted and cleared. This facility is a two-story home that consists of 3 bedrooms, 3 bathrooms, kitchen, living room, dining room, attached garage, front yard and backyard fenced.

Areas off limits to children include- entire 2nd story of the home, attached garage, front yard.

Areas on limits for the children include- daycare area (Livingroom), kitchen, restroom (1st floor), back yard fenced.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA Garcia inspected the facility for safety, comfort, cleanliness, ventilation and working phone (cell phone). For ventilation, LPA Garcia observed an in-wall A/C unit located in the Livingroom. LPA observed the furniture, children’s materials, to be in good condition and age appropriate. LPA observed the children to have storage space to place belongings and utilize cots for nap time. Licensee stated naptime takes place in the main daycare area of the home.

LPA Garcia asked about cleaning compounds, licensee stated they are stored in the attached garage. Knives were observed to be stored in a high kitchen cabinet making it inaccessible to the children. For drinking water, licensee stated she provides filtered water the children and the children bring their individual water bottles from home. Licensee does not currently participate in a food program.

LPA Garcia entered bathroom (1st floor) accessible to the children, and observed the toilet, hand washing sink, hand soap and changing station. LPA did not observe any hazardous materials. LPA observed the restroom to be clean and in operable condition.

LPA Garcia asked the licensee if there were any pets, poisons, firearms, or weapons. The licensee stated there are no pets, no poisons, no firearms, and no weapons. LPA did not observe poisons, firearms, or weapons. The licensee was informed that if any poisons (ex; Drano, rat poison or items that fall into that category), firearms and weapons are purchased, it is required to be locked with a key or combination lock and firearm and ammunition must be stored separately.



LPA Garcia inspected the back yard and observed toys and other age appropriate materials for the children. The back yard is surrounded by 3 property walls, 2 sides are wooden gate and the other is a concrete brick wall. LPA Garcia observed a wooden wall container below the wooden fence to appear unstable and appeared to be falling. However, when LPA Garcia attempted to shake or move, it feels very sturdy and does not move. LPA took photos of the wall container, and licensee stated that the fencing is also being replaced and the container will also be repaired. LPA Garcia observed a cover for the air conditioner making it inaccessible to the children. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards (cement). LPA did not observe, sharp or loose, pointed parts. For outdoor water drinking, licensees bring out individual water bottles and refilled as needed.

LPA Garcia observed the required fire extinguisher located in the located in the closet. Licensee stated she bought the fire extinguisher in fall around august or September. LPA advised licensee that the fire extinguisher must be re-purchased or serviced on a yearly basis. LPA observed the dual smoke/carbon monoxide detector in the kitchen. The licensee tested the dual carbon monoxide and smoke alarm. LPA Garcia heard the sounds and are operable. LPA observed the first aid complete. For ill isolation, licensee stated the children have a quiet corner for ill isolation until parents pick up the child. LPA observed current facility roster.

LPA Garcia observed licensee Pediatric First Aid/ CPR certification dated 10/11/25. Health and Safety certification is dated 12/16/22. Child Abuse Mandated Reporter (AB 1207) is dated 10/1/25. LPA did not observed proof of immunizations for licensee against Pertussis, Measles, TB and Influenza. Licensee was advised that the mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Corrections: Licensee will provide photos of new wooden fencing that is being replaced and provide photos to LPA Garcia when finished (estimated date Monday June 8th).

An exit interview was conducted, and a signed copy of this report and appeal rights was provided to the licensee, Jenik Hairapetian.
