On August 6, 2025 Licensing Program Analyst (LPA) Evelyn Garcia conducted an annual unannounced inspection. Upon arrival, LPA met with licensee Mariana Tataryan. There were 13 total children sleeping upon arrival and 1 other adult helping to provide care for the child. The fire department has granted a fire clearance. Other residents of the home include licensee, husband, and two minor child. LPA confirmed with licensee that all adults residing in the home have criminal record/TB clearances. The facility will operate Monday – Friday 7:30am-5pm and with a license capacity of 14 children. provided licensee a copy. LPA reminded licensee about child ratios for large fcch which are to operate with a Maximum of 14 children in care with no more than 3 Infants, also must have 2 school age children enrolled. Or Max. 12 Capacity with no more than 4 infants.

Physical Plant: This is a single story home that consists of 2 bedrooms, 1 bathroom, living space, kitchen, front and backyard fenced. This is a front home property and the back home has a separate address and is gated off from the front property. The home was toured; the following areas are used for daycare: living room, bedroom #1(Play/naproom), bathroom and front yard.

Off limit areas include: Backyard, kitchen, bedroom #2. Drawers and lower cabinets in kitchen/bathroom have magnet locks and do not contain any hazardous items. There is an operational dual smoke and carbon monoxide alarm and fire extinguisher maintained in the home along with the pull down fire alarm station. The home has electrical outlet covers throughout and maintains a First Aid Kit in the kitchen top cabinet. There are adequate age-appropriate toys, books, games, and napping cots located in bedroom that children use a play and naproom. The outdoor play area is gated with safety gates around the area that contains the artificial turf and climbing equipment.The front yard is free of hazards and has sufficient toys.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There are no firearms present in the home and there is no smoking on the premises as stated by Licensee. The dual smoke detector and carbon monoxide detector are in operable condition. LPA observed the required fire extinguisher (2A10BC) is fully charged and is in the green area indicating it is fully charged and purchased 8/4/25 with receipt attached. The facility sketch is complete and current, there is working telephone (cell). Fire/Disaster Drills document are maintained current and was last conducted on July 12, 2025.

Safe and age appropriate toys, play equipment and materials were observed. Electrical outlets are inaccessible. LPA reminded licensee, no baby bouncer saucer chairs, or any recalled and or prohibited toys or sleep/play equipment are allowed on the premises. There is age appropriate napping cots and cribs. There are no window cords accessible to children.

Bathroom: Bathtub and shower are free of hazards. Cleaning supplies are kept in a high kitchen cabinet with safety lock above the stove, and kitchen is inaccessible to the children.

Kitchen: The home has a clean and fully stocked clean refrigerator/freezer. Breakfast, lunch, snacks are provided and parents will have the option to bring their own food from home. Licensee is advised food brought from the children’s home shall be properly stored, labeled, and dated. Licensee is a participant in a food program.

Outdoor: Children play in the front yard. The play area is clear and clean of debris, play area is fenced and gated all around, play yard has artificial grass and cement for play time. There is climbing equipment that is placed on artificial turf and provides some cushioning to minimize injuries. LPA observed a storage shed in the front yard which is locked with a keypad lock. LPA observed age-appropriate toys for children.

Advisory/Other: First Aid kit was observed with supplies (thermometer) readily available and kept in the kitchen cabinet. Licensee has a valid CPR/First Aid certification expires 7/2026, and Mandated Reporter is current and dated 12/13/23 and her assistant’s dated, 3/29/2025. Licensee is reminded First Aid/CPR and Mandated Reporter must be renewed every two years.

The following was also discussed with the licensee:

1. In the absence of the licensee a qualified adult must be present, supervising the children; a qualified adult is an individual who has a valid and current Pediatric first aid/ CPR-adult-child- infant certification (EMSA approved), a valid criminal record clearance associated to the facility license, immunization's (MMR, TDAP, TB and Influenza or Influenza declination), AB 1207 Child Abuse Mandated Reporter Certificate.

2. A current roster of children enrolled must be available and maintained for a period of 3 years, even after children are no longer attending the facility.

4. Annual fees must be paid promptly and by the due date or a late fee shall be assessed and/or the license shall be terminated.

5. The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary and smoke and carbon monoxide detectors should be checked, and batteries replaced as needed.

6. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your home.


7. Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing (refer to LIC 624B). Mandated reporter requirements were reviewed and explained.

8. Fire and safety drills must be performed every six (6) months and documented for review by the Department.

9. Smoking is prohibited in the family childcare home.



10. Children and staff records must be maintained and updated as needed and be available for review by the Department.

11. Immunization Requirement: H&S 1597.622: Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. The licensee and all adults working with children have proof of immunizations.

During the exit interview, the LICENSEE Mariana Tataryan, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

A notice of site visit was given and must remain posted for 30 days.

There were no deficiencies cited at today's visit.

LPA issued 2 Advisory Notes for LPA to include ID for a child's file and to have a facility roster available upon request.
