On 2/06/2025, Licensing Program Analyst (LPA) Carol Heath unannounced an annual inspection with the licensee, Gretchen Toledo, to ensure the facility meets licensing requirements. The licensee is requested to provide care for a large family childcare home with a capacity of 14 children. LPA toured the house in and out. Individuals who reside in the home include 2 adults (licensee, husband) and 3 children (10 days old and 2 years old daughters , 5 years old,son). Per Guardian, all adults in this facility obtain a criminal record clearance.
During today's inspection, there were 6 children (0 to preschool age) with the licensee. The hours of operation are Monday through Friday, 6:00 am to 6:00 pm. The incidental Medical Services (IMS) policy was discussed. The licensees will not provide the IMS at this time.
The home is described as follows:
This two-story home consists of a 6-bedroom, 5-bathroom home with a kitchen, childcare (formal dining area), living/dining room, laundry, and garage. There is no body of water on the premises. Childcare children can access the backyard and kitchen.
Indoor:
Main area: Main care is provided in the Living room used for childcare. Children use the bathroom on the left in the hallway. Living room: A fireplace in the Living room is screened to make it inaccessible to the children. LPA observed a cover in front of the fireplace, which was made inaccessible to children. PA observed many age-appropriate toys, books, and other materials in the Dining room area.

Per the licensees, the children will nap in the Living Room. Bathroom #1: Children will use bathroom #1 next to the laundry room. Bathroom #1 was toured and inspected. The toilet is in operable condition. The faucets are clean, safe, and operable.
Kitchen/Dining: All sharp utensils, cutlery, cleaning supplies, medicines, drawers, cabinets with plastic bags, and pointy things or small things children can swallow are inaccessible to children with a child safety latch under the kitchen sink. The refrigerator, dishwasher, stove, microwave, etc., are clean. The kitchen was clean, orderly, and free of hazardous items. Medications were stored in the off-limits bedroom.
Outside: The backyard was inspected. The backyard is completely fenced. The backyard is completely fenced (brick). There are age-appropriate toys. LPA observed a swing set anchor on the ground. LPA also reminded the licensee to supervise children at all times while playing in the backyard. Both site yards are inaccessible to the children with a gate.
Off-limit areas: Laundry, Garage and all second-floor bedrooms and bathrooms (safety gate).
Other:
AC/Heating Unit was observed. The AC/Heating Unit is located on the right side of the home and is inaccessible to children with a gate.
Bodies of water: Pre the licensee, there were no bodies of water in the home. There is a decorative water fountain that has no water.
Electrical outlets: All unused electrical outlets are plugged in and inaccessible to children.
Food: The licensee enrolling in the Food program. The licensee will provide Breakfast, lunch, and snacks.

Fire extinguisher (2A10BC): LPA observed a required fire extinguisher (2A10BC) reading in Green, located in the kitchen, and inaccessible to children. It meets standards established by the State Fire Marshall.
Fireplace: The fireplace was observed in the living room and is screened to make it inaccessible to the children.
Hanging window blind cords: The cords are inaccessible to children.
Isolation area (Illness): Per the licensee, if the child shows signs of illness, they will be separated from other children and stay near the classroom area.
Medications and cleaning solutions: Detergents/cleaning compounds are in the laundry cabinet. Medications are in the off-limits bedroom.
Napping: Children will nap in designated areas with adult supervision. LPA observed 6 cots ,5 mats and 4 playpens in the closet.
Overnight Care: According to the licensee, it does not provide overnight care.
Pets: There are 1 small dog (in the kitchen). The dog has current vaccinations.
Phone service: There is a working cell phone.
Smoke Detectors and Carbon Monoxide: The smoke detectors and carbon monoxide devices tested operable.
The First Aid kit is in the kitchen cabinet, inaccessible to children. The First Aid Kit was observed to be complete with supplies and a first aid manual.
Transportation: The licensee does provide transportation for children. The licensee has a valid California driver's license, valid vehicle insurance, and vehicle registration.
Weapons or Firearms: Per the licensee, there are No Firearms at the facility at this time. LPA does not observe any firearms.

Documentation:
The licensee has current CPR and First Aid Training with the expiration dates of 06/2026 and Prevented Health and Safety Training completed on 3/13/2021. The licensee has her fingerprint clearance and TB exam. The licensee has proof of being immunized against influenza, pertussis, and measles.

Documentation Review:
Child Files: LPA reviewed 4 children's records. Some files are missing information. There are 2 infants enrolled in the facility. The files are missing LIC 9227: the licensee did not have LIC 9227 forms or sleeping logs for two infants. LPA shared information about Safe Sleep Regulations with the licensee.
Staff Personnel File: During the inspection, LPA reviewed 1 staff file, which included LIC 508, LIC 9052, immunization records, a TB test, LIC 9108, Mandated Reporter Training, and CPR/First Aid certificates. All required documents were in place.
Immunization Records: The licensee and her assistant have up-to-date immunizations for MMR and DTaP, and both have submitted written statements declining the influenza vaccine.
CPR/First Aid: LPA verified that the licensee and her assistant hold current Pediatric CPR and First Aid certifications, which expire on 7/2026 and Mandated Reporter Training: The licensee and her assistant completed the online mandated reporter training at www.mandatedreporterca.com, which expires on 7/2024.
Facility Fees: According to the Licensing Information System, the annual facility fees are current.
Fire/Disaster Drill: The licensee conducts fire and disaster drills every six months, with the last drill documented on 06/03/2024.

Required Postings: LPA observed that the licensee has the Facility License, Emergency Disaster Plan, Earthquake Preparedness, and Parents' Rights Poster displayed as required.
The following information was discussed with the licensee:
o A baby walker shall not be allowed on the premises of a family childcare home in accordance with Health and Safety Code sections 1596.848(b) and (c). State law prohibits baby walkers, bouncy seats, exersaucer, and other items that fall into that category.
o The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B
o Licensee was advised to visit the CCL website (www.ccld.ca.gov) to obtain updates on courses and updates/changes to the regulations. Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. The licensee was advised that the inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility's phone number; if the phone number is changed, licensing must be notified.
o Requirements for fire drills, earthquake drills, and documentation for both.
o The Duty Worker is available for questions Monday through Friday at (661) 202-3318 from 8:00 a.m. - 5:00 p.m.
o The licensee is reminded that 100% supervision is always required for children.
o The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hotline at 1-800-540-4000. Also, contact the CCL office and follow up with a written Unusual Incident/Injury Report (LIC 624B).

o The regulation prohibits tobacco smoking in private residences that are licensed as family childcare homes and in those areas of the family childcare home where children are present (24/7 ban).
ü LPA discussed the safe sleep regulations with the licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and- resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
ü Licensee [or facility representative] was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
